Job Summary:
We are seeking an advanced practice provider (APRN or PA) with experience in primary care or internal medicine, preferably with a focus on geriatrics. As an AP, you will collaborate with a team of providers and case managers to improve outcomes for long-term care residents. You'll enjoy a great benefit/pay package and possibilities for career growth.
Duties and Responsibilities:
- Assess patients' medical and mental health needs and provide a plan of care that proactively manages their conditions and addresses barriers to care
- Ensure compliance with local, state, and federal agencies related to clinical services
- Prescribe medications and order lab work, diagnostic procedures, and consultations
- Monitor patients' compliance and response to treatment and modify those plans as needed
- Work with RN case managers as part of an integrated care team
Requirements:
- FNP, AGNP, AHACNP or PA license required
- Master's degree from an accredited APRN or PA program
- 3 years' experience in clinical nursing or rehab in geriatric populations
- Electronic Health Records experience
- Working knowledge of Microsoft applications, including Word, Outlook, and Excel
Estimated Salary: $120,000 - $150,000 per year
