Job Summary
We are seeking a dedicated and compassionate Registered Nurse Case Manager to join our healthcare team. The Registered Nurse Case Manager will work collaboratively with healthcare providers to enhance patient service and outcomes.
Duties
- Conduct comprehensive patient assessments to determine individual healthcare needs.
- Develop and implement personalized care plans in collaboration with interdisciplinary teams.
- Maintain accurate medical records in compliance with HIPAA regulations.
- Monitor patient progress and adjust care plans as necessary to ensure optimal outcomes.
- Serve as a liaison between patients, families, and healthcare providers to facilitate communication and understanding of hospice care.
- Educate patients and their families about health management and available resources.
- Participate in quality improvement initiatives to enhance patient care delivery.
Requirements
- Current Registered Nurse (RN) license in the state of practice.
- Proven experience in hospice RN case management.
- Strong knowledge of medical records management and patient assessment techniques.
- Familiarity with HIPAA regulations and commitment to maintaining patient confidentiality.
- Excellent communication skills with a focus on patient service.
- Ability to work collaboratively within a multidisciplinary team environment.
- Critical thinking skills and the ability to make informed decisions regarding patient care.
- Certification in case management is preferred but not required.
