Job Summary:
We are seeking to add a Case Manager to our New Mexico team! The RN Case Manager is pivotal in managing and coordinating the care of patients with complex and chronic medical conditions. They are dedicated advocates, ensuring patients receive the most appropriate and timely services and resources. Through close collaboration with the care team, patients, their family, team leads, nurses, HHA's, and FHHA's to develop and execute comprehensive plans of care, schedule appointments and services, facilitate communication between team members, and closely monitor patient progress.
Five Major Job Functions:We are seeking to add a Case Manager to our New Mexico team! Through close collaboration with the care team, patients, their family, team leads, nurses, HHA's, and FHHA's to develop and execute comprehensive plans of care, schedule appointments and services, facilitate communication between team members, and closely monitor patient progress.
- Lead, Manage, Drive Accountability for training Team Leads and Home Health Aide field staff
- Assess patient need and develop individualized care plans
- Conduct assessments for ongoing evaluation and advocacy of patient's level of care, utilization, and services provided
- Provide direct patient care, ensure timely documentation, and coordination of care
- Collaborate with clinical leadership team with patient care, rounding, and care conferences
Requirements
- Current New Mexico or compact Nursing License
- Current BLS/CPR certification
Additional Information:
- We offer competitive salary of $85,000-$95,000, health benefits, unlimited PTO, 401k
