We are looking for three full time case managers to cover the following shifts:

  • (2) 7a- 3:30p
  • (1) 3p- 11:30p

FLSA: Non-Exempt  

 

Location: Portland, OR

On October 14, 2024, Tuerk House contracted and opened Multnomah County’s Coordinated Care Pathway Center in Portland, Oregon. This now adds further support, evidence- based services, and care to the community's public health crisis. The Center will also help to address concerns about public safety, livability, and overall community health and well-being.

JOB SUMMARY/OBJECTIVE:  

 

The Case Manager will be responsible for providing case management services to clients seeking deflection and sobering services at the Coordinated Care Pathway Center (CCPC). This position will work in collaboration with a multidisciplinary team to assess the needs of the individuals participating in deflection or sobriety services. The goal of the case manager within the deflection/sobriety center will be to offer a plan for individuals to access needed medical, social, educational, substance use, mental health, and other applicable services. Case managers contact and interact with a variety of multiple constituencies such as law enforcement, emergency responders, community -based organizations, Multnomah County staff, and the general public. 

 

ESSENTIAL DUTIES AND RESPONSIBILITIES: 

  • Collaborate with clinical staff to identify individuals in need of deflection and sobering services and case management support.
  • Conduct thorough evaluations to determine clients' needs, including support related to housing, substance use treatment, vocational skills, and mental health services.
  • Assist clients in applying for essential public services, benefits, and support programs, ensuring eligibility and proper documentation.
  • Develop and maintain an updated list of local community resources and agencies that provide essential services, including housing, health care, financial assistance, and substance abuse treatment.
  • Create comprehensive individualized care plans that address clients' specific needs, including medical care, substance use treatment readiness, medications, legal issues, financial assistance, and mental health support.
  • Discuss comprehensive individualized care plans with clients, educate clients on eligibility criteria and completion criteria
  • Work closely with medical and support staff to gather relevant client information, such as assessment results and medical records, ensuring smooth continuity of care and discharge planning.
  • Make appropriate referrals to address clients' broader social service and support needs, including housing, mental health care, legal assistance, and financial help.
  • Work collaboratively with PATH team to coordinate care, monitor clients' progress, assess the effectiveness of the service care plan, and make adjustments as needed to ensure optimal outcomes.
  • Assist clients in finding long-term solutions to immediate financial and housing crises, linking them with emergency resources and support services.
  • Actively participate in required team meetings, training sessions, and professional development opportunities to stay current on best practices and regulatory requirements.
  • Collect and submit relevant data on client outcomes, services rendered, and program performance for monthly reporting and program evaluation.
  • Review and comply with Tuerk House policies as outlined in the employee handbook
  • Perform other duties as assigned within the scope of the position, ensuring flexibility and adaptability to client needs and program changes.

REQUIRED EDUCATION, CERTIFICATIONS AND/OR EXPERIENCE:  

 

  • Bachelor's degree in social work or related field required
  • Preferred two (2) years' experience as a Case Manager 
  • Adheres to HIPAA and state compliance regulations.
  • EMR Compliance: Ensure compliance with electronic medical record standards and uphold the 48-hour documentation requirement.
  • BLS Certification (Required or the ability to obtain within the first 90 days of employment).

 

REQUIRED KNOWLEDGE, SKILLS AND ABILITIES:  

  • Substance Use Knowledge: Extensive knowledge of psychiatric illnesses and substance abuse disorders, including treatment options and psychopharmacology.
  • Community Resources: Familiarity with community resources, networking techniques, and emergency stabilization protocols.
  • Computer Proficiency: Proficient in computer skills, including email communication and documentation.
  • Communication Skills: Excellent communication skills in English, with active listening abilities.
  • Documentation: Expertise in documenting medical records accurately and comprehensively.
  • Organizational Skills: Strong organizational and time-management abilities, with attention to detail.
  • Personal belief in recovery and the recovery process.
  • Excellent written and verbal communication skills, with the ability to interact effectively with staff, clients, and the public.
  • High energy, flexibility, and strong interpersonal skills to work effectively within a team.
  • Ability to engage clients flexibly based on their individual needs.
  • Respect for and maintenance of professional boundaries with all individuals.
  • Ability to follow written and verbal instructions accurately.
  • Proficiency in reading, understanding, and speaking English.

 

Background: This position requires successful background and drug test completion  

 

Physical Requirements: 

  • Prolonged periods of sitting at a desk, working on a computer, or walking.
  • Must be able to lift up to 15 pounds at times.

The compensation for this position will vary depending on factors such as your location, skills, job-related knowledge, and experience. The compensation package may also include incentive compensation opportunities in the form of discretionary annual bonus.
