Certification and Experience Requirements

- KY License Mandatory
Job Responsibilities
- At least 2 years of Inpatient PT experience in an acute care setting.
- Responsibilities will cover IP Acute care, SNF, and LTACH.
- Orientations take place bi-weekly.
Assignment Details
- Available start date as requested by the job seeker.
- Resource must reside within 100 miles of the facility to be considered.
- Confirmation of assignment must be loaded within 24 hours of accepting the offer.
- Driver's license is required for submission.
Job Requirements
- Possess KY License and BLS Certification.
- ACLS certification is optional for RNs except Psych and True Med/Surg units.
- Provide proof of MMR vaccinations or positive titer prior to start.
- Acceptable Hep B vaccine series or declination.
- Varicella vaccinations or positive titer are required prior to start.
- Td vaccine required.
- If positive TB record, provide Chest X-Ray with questionnaire.
Schedule Information
- Active from September 1st to May 31st, subject to change at the facility's discretion.
- Religious and Medical exemptions accepted with proper documentation and approval.
Unit Specific Information
- Cover sheet and requirements must include facility and unit information: minimum of 2 years dependent on specialty.
Additional Details
- Two references required, one being a supervisor reference within the last year.
- Annual update mandatory for various credentials.
