What are the responsibilities and job description for the Outreach Program Coordinator position at Tulsa Day Center?
Primary Roles & Responsibilities
· Work collaboratively with Director of Outreach to ensure Outreach Specialists are providing housing-focused case management to people experiencing homelessness.
· Develop reference materials about processes essential to achieving housing plans: re-establishing vital documentation, identifying housing opportunities, internal Outreach procedures, etc.
· Assist in the hiring and onboarding of Outreach team members.
· Assist in the organization of the on-boarding process for new Outreach team members.
· Help the Director of Outreach generate and formalize procedures for Outreach team to ensure efficiency and goal completion.
· Coordinate with Outreach Director to locate regular training opportunities for field Outreach teams.
· Help Director of Outreach monitor Outreach Team progress towards benchmarks for overall performance goals of Coordinated Entry System (CES) achieving positive outcomes for outreach clients.
· Ensuring the maintenance of complete and confidential client records within HMIS.
· Monitor and aid in documentation of hotel usage ensuring proper program entry in HMIS, usage of service transactions within profiles and alignment of final billing from the hotel provider with data collected.
· Participate in weekly staffing's with Director of Outreach and Outreach Specialists to identify housing plans, troubleshoot barriers, and ensure progress for all clients enrolled in TDC Outreach Case Management.
· Assist with the regular evaluation of entire caseload with Director of Outreach.
· Help monitor data quality standards for client information in the Homeless Management Information System (HMIS) in coordination with the Director of Outreach.
· Help Outreach Specialists prepare referral packets to Permanent Supportive Housing and Transitional Housing programs, as needed.
· Maintain data representing client interaction with service providers, housing providers, and vital document providing agencies.
· Help Outreach Specialists in accessing resources and benefits—such as vital documents and income verification—and making referrals.
· Help facilitate communication between Outreach team members and service providers regarding client updates.
· Forge and maintain positive, productive working relationships with partner agencies and other providers of services and resources including law enforcement, City of Tulsa, and private partners within the business community.
· Attending team meetings, training workshops, and community meetings with the Director of Outreach when needed.
· Other duties as assigned.
Qualifications
· Bachelor's degree in social work or related field or relevant experience preferred.
· Previous supervisory or team lead experience.
· Experience with program implementation and development preferred.
· Experience working homeless service providers in some capacity preferred.
· A valid driver’s license in the state of Oklahoma is required.
· Demonstrated ability to work independently and as part of a team.
· Knowledge of local social services and community resources.
· Strong organizational and time management skills.
· Adapts well to changing priorities and challenging individuals.
· Strong written and verbal communication skills.
· Exercises good judgement and discretion in handling client confidentiality and other confidential matters.
· Possesses a valid Oklahoma driver’s license, proof of insurance, and a clean driving record.
· Computer skills with proficiency in Microsoft office programs including Excel, Word, Outlook, and OneDrive, as well as Adobe Acrobat.
· Familiarity with Harm Reduction and Housing First models of service delivery preferred.
Job Type: Full-time
Pay: $50,000.00 - $55,000.00 per year
