The mission of the Tunnel to Towers Foundation is to honor the sacrifice of firefighter Stephen Siller, who laid down his life to save others on September 11, 2001. We also honor our military and first responders, who continue to sacrifice life and limb for our country. The Tunnel to Towers Foundation is committed to eradicating homelessness among our veterans.
The Foundation seeks an experienced Case Manager to collaborate with and oversee the program staff serving Veterans who have experienced unsheltered homelessness and are now transitioning into permanent housing.
We seek a dedicated, experienced Case Manager to join our Supportive Housing Program. The ideal candidate will have a strong background in social work or a related field and will be committed to providing comprehensive case management services to individuals needing supportive housing. This role requires excellent organizational, communication, and problem-solving skills to support our Veterans in achieving stability and self-sufficiency.
Duties and responsibilitie
- sProvide comprehensive case management services to Veterans, including assessments, service planning, and ongoing support
- .Develop individualized service plans based on Veterans' needs, goals, and strengths
- .Monitor and evaluate Veterans’ progress, adjusting service plans to ensure adequate support
- .Effectively utilize Electronic Health Record (EHR) systems to document case notes, service plans, and all relevant Veteran interactions
- .Coordinate with community resources, service providers, and support networks to meet Veterans' needs
- .Conduct regular visits and check-ins to assess living conditions, wellness, and compliance with program expectations
- .Advocate for Veterans to ensure access to necessary healthcare, employment, and benefits
- .Maintain accurate and up-to-date case files, documentation, and reports, ensuring data integrity and confidentiality
- .Facilitate life skills workshops, employment readiness training, and financial literacy sessions
- .Collaborate with multidisciplinary teams, including entitlement and employment professionals, behavioral health specialists, peer support, and property management, to support Veterans' stability and development
- .Provide crisis intervention and de-escalation support, connecting Veterans with emergency resources when needed
- .Assist Veterans with navigating housing retention strategies, eviction prevention, and conflict resolution
- .Ensure compliance with program policies, procedures, and regulatory requirements while maintaining a strengths-based and trauma-informed approach

- nsBachelor’s degree (B.A.) in Social Work, Psychology, Human Services, or a related fiel

- d.Minimum of 3 years of case management experience in a supportive housing, social services, or related environmen
