Position Overview:
As a Hospice Nurse (RN/LPN) with Turas Hospice & Palliative Care, you will play a vital role in providing exceptional care to patients on their end-of-life journey. Working alongside our multidisciplinary Journey Team, you will offer support, comfort, and medical care in the home, facility, or other assigned settings. Key Responsibilities:
- Direct Patient Care: Provide hands-on, compassionate care to hospice patients in their homes or hospice facilities.
- Assessment & Monitoring: Conduct nursing assessments, monitor vital signs, and evaluate patient conditions to ensure comfort and symptom management.
- Medication Management: Administer prescribed medications, including controlled substances, and monitor their effectiveness and side effects.
- Care Planning: Collaborate with the Journey Team, including physicians, social workers, chaplains, and volunteers, to develop individualized care plans tailored to the patient's needs.
- Patient & Family Education: Offer emotional support and provide families with guidance on care options, medications, and available resources.
- Documentation: Ensure all patient records are accurate and updated in accordance with Ohio state regulations and company policies.
- On-Call Support: Participate in on-call rotations to provide consistent care and support to patients after hours as needed.
Qualifications:
- Active LPN or RN License that is in good standing with the state of Ohio
- Previous hospice or palliative care experience is preferred but not required.
- Strong communication, organizational, and interpersonal skills.
- Ability to work independently and collaborate with a multidisciplinary care team.
