Position Overview:
We are seeking a compassionate and dedicated Care Coordinator to join our team. The Care Coordinator will play a vital role in facilitating and coordinating care for individuals undergoing substance use recovery. They will work closely with clients, healthcare professionals, and other stakeholders to ensure seamless coordination of services and support throughout the recovery process.
We’re looking for a bright, energetic, and flexible clinician who can screen, assess, and advocate effectively for clients in a system that includes medical, counseling, and peer professionals. Job Responsibilities:
Screening and Assessment: Screens and assesses potential clients to determine the severity of Substance Use Disorder (SUD) and Co-Occurring Mental Health to ensure appropriateness of care. Perform biopsychosocial evaluations to assess client’s needs in treatment.
Level of Care Placement: Coordinate with medical and clinical team to determine client’s appropriateness for treatment and level of care determination as indicated by the screening and assessment.
Client Orientation: Performs new client orientation and reviews New Client Handbook, guidelines, program requirements and expectations with client.
Family Orientation: Provides incoming client’s family members with information necessary to participate in the Family Program.
Case Management: Perform case management assessments and help build treatment plans to assist clients with case management needs in conjunction with the peer support team. Present findings and staff in treatment team meetings. Maintain accurate and up-to-date documentation of client interactions, care plans, and progress notes in accordance with organizational policies and regulatory requirements.
Treatment Planning: Work closely with client to develop individualized treatment plan and SMART goals integrating the client’s strengths, needs, abilities and preferences
Care Coordination: Coordinate client care with primary care physician, primary therapist, medical technicians, peer support specialist and other outside services as indicated by the client treatment plan.
Client Education: Inform clients about the various approaches, tools and skills that will assist them in their recovery process.
Client Counseling: Assist clients in motivation and self-discovery utilizing clinical interventions.
Documentation, Electronic Health Records (EHR): Complete and document client progress in the EHR as defined by agency Policies and Procedures.
Crisis Management: Assist clients in distress, guiding toward emotional regulation and stabilization and care coordination when indicated.
Customer Service: Provide guidance on how to navigate the agency’s program services and guideline and assist clients in connecting to the proper staff member to address questions as they arise. This position has direct contact with clients, family, and other supportive networks.
Collaborative Services: Participate in multidisciplinary team meetings to discuss client progress, treatment plans, and any challenges or barriers to clients recovery.
Job Skills Maintenance: Stay informed about developments in the field of substance use recovery and participate in training and professional development activities as needed.
Qualifications:
Master’s degree in social work, counseling, or public health.
Strong understanding of the ASAM criteria and its application in individual and group therapy settings as well as inpatient services.
Ability to assess clients using the multidimensional ASAM criteria to determine appropriate level of care and to develop individualized treatment plans.
Proficiency in writing clear and concise treatment plans, progress notes, and case summaries according to ASAM guidelines.
Minimum of 2 years of case management and assessment experience working in the substance use recovery field or a related healthcare setting.
Strong understanding of substance use disorders, treatment modalities, and recovery principles.
Excellent interpersonal and communication skills, with the ability to build rapport and trust with clients from diverse backgrounds.
Demonstrated ability to work collaboratively in a team environment and effectively coordinate care across multiple providers and agencies.
Ability to maintain confidentiality and adhere to ethical standards in accordance with professional codes of conduct.
Proficiency in computer skills, including electronic health record systems, Lightning Step, Microsoft Office Suite, Google Workplace, and Adobe PDF Maker.
Valid driver's license and access to reliable transportation.
Education/Certification/Experience Requirements:
Masters in Social Work- LMSW or LCSW; counseling, or public health.
Case Management Certification - ACMA, ACM, or CCM certified.
Minimum of 2 years of case management and/or assessment experience required

Physical Requirements:
The physical requirements of the position include but are not limited to, standing, walking, sitting for long periods of time, use of hands to finger, reach with hands and arms, balance, stoop, and squat.

Must be able to lift and carry approximately 15 pounds.
Must be able to speak English and listen, hearing sound and words.

Salary : $55,000 - $70,000
