Position Summary
The QMHS Case Manager for Outpatient Services is a member of the Youth or Adult Clinical Team. is responsible for providing Behavioral health case management services including Therapeutic behavioral services (TBS), Psychosocial Rehabilitation (PSR), Community Psychiatric Supportive treatment (CPST), and/or SUD case management services in community locations based on the needs of the persons served. This is an hourly position with hours and schedule established to complete required duties. This is a clinical position, as defined by the U.S. Department of Labor and, as such, is not “exempt” from wage and hour regulations. The QBHS continuously works with the management and administrative team to assure that all programs are in compliance with the overall objectives and are in accordance with the policies and procedures set forth by the Ohio Administrative Code, ODJFS, ODCYS, CARF, OHMAS, etc. and the Board of Directors.
Essential Duties/Responsibilities
The following Principal Accountabilities are the essential functions of this position. Successful job applicants will be able to perform these essential functions with or without requested reasonable accommodation.
1. Maintain an average 34 billable hours a week/ 6 hours of productivity.2. Once licensed, provide support as needed for Youth and Adult client services.3. Perform duties in accordance with applicable professional ethics and standards as well as established Turning Point Behavior Health Services’ policies.4. Comply with all state and federal laws regarding protected health information. 5. Collaborates within the Integrated care team approach to case management, ensuring that goals established by the client are carried out within time frame of treatment plan.6. Ensures complete and timely documentation in client record including progress notes, referrals, and releases of information. All Documentation must be completed within 48 hours of the service.7. Provide education and support to clients, family members and peers regarding behavioral health, symptom management and wellness.8. Provide Community Outreach to connect community needs and available resources with the agency.9. Develop partnerships with referral and funding sources.10. Attend required staff meetings, training, and individual or group supervision as scheduled.11. Facilitates and co-facilitates psychoeducational and substance use group including but not limited to: Partial Hospitalization, Intensive Outpatient, and Day treatment.12. Provide direct clinical services and ongoing assessment/monitoring to active Turning Point Behavioral Health Services’ clients. Communicates changes to Integrated care team to ensure treatment plans are updated and appropriate interventions are provided.13. Provide community-based services such as obtaining/maintaining benefits, medical appts, housing, food, vocational and other needs indicated in treatment plan.14. Review and follow policies and procedures of the agency.15. Complete required trainings to maintain agency’s compliancy.16. Maintain an approved set work schedule between the hours of 8:00AM- 5:00 PM, with a ½ an hour lunch, based on the agency’s hours. Unless otherwise approved by Supervisor.17. Maintain accurate time records.18. Provide written/oral reports as directed or as necessary. 19. Work collaboration with Administrator on agency compliance with ODJFS, ODCYS, CARF, OHMAS, etc. and regulations of the administration code.20. Any other tasks that relate to the overall planning, leading, and organizing of the
facilities for Turning Point Residential Services.
Essential Knowledge, Skills, Abilities and Personal Characteristics
- Provide clinically appropriate services that address behavioral health concerns of adult and/or youth clients. * Maintain licensure(s) and certification(s). * Practice effective treatment models as directed by the agency. * Work as part of multidisciplinary team. * Collaborate professionally and effectively with external providers. * Possess a working knowledge of community resources and how to access them. * Possess a working knowledge of child/adolescent development, cultural competency, behavioral health and substance use disorders/treatment and assessment.
Minimum Qualifications:
- Must be 21 years of age. * Must complete training for status of QMHS to provide mental health services. * High School diploma and Chemical Dependency Counselor Assistant (CDCA) if applicable * Must have demonstrated experience in healthcare, rehabilitative, or other behavioral health setting. * Six months to one years’ experience as a youth care workerpreferred. * Knowledge and understanding of youth with behavior issues. * Oral and written communication skills * Knowledge and skills in educating youth in daily living skills. * Observation and decision-making skills. * Computer skills and proficiency with Microsoft Office suite software a must; experience working with Electronic Health Records systems (EHRs).
Experience Qualifications:
- Direct experience with at-risk populations for a minimum of 2 years. * Experience in harm reduction, safety planning and crisis intervention. * Demonstrated ability to work co-operatively and negotiate effectively within a multi-disciplinary team of service providers. * Experience in working with diverse cultures and communities. * Knowledge of and sensitivity to the life experiences and needs of the youth and adult community combined with previous youth and adult outreach and/or community development experience. * Candidates without the specific education qualifications and where the position does not require certified qualification to practice, but who possess an appropriate combination of experience and other academic qualifications will also be considered.

- State of Ohio driver’s license (clean driving record) or identification in good standing * CPR and First Aid certification.
**Must maintain required re-certifications of all of the above.
Physical Demands
Work Environment
While performing the essential duties of this job, the employee may be required to climb and/or lift.
Works in residential group home facility or behavioral health setting with limited on-site supervision. Some program activities are conducted in community settings away from Turning Point Residential Servicespremises. Work conditions, such as cleanliness, comfort and safety, may vary widely. Exposure to all types of weather elements likely. May be scheduled to work evenings, weekends, Safety management and crisis intervention decisions and actions are dependent on staff present. Management of emergencies is dependent on staff present and/or outside emergency services. Staff subject to hostile and emotionally upset youthor adults, family members, staff, visitors, etc. under all conditions/circumstances. A crisis could arise which may be stress producing and/or exhausting.
Conditions of Employment
