JOB TITLE : CNA
DEPARTMENT : Home Care Agency
REPORTS TO : Home Care Director
POSITION TYPE : 12 hour shift - 8am - 8pm Every other weekend
CLASSIFICATION : Non-Exempt
POSITION SUMMARY :
Responsible for assisting Home Care Agency clients with personal care and homemaker needs.
Essential Duties and Responsibilities :
- Use appropriate skills and abilities to support Home Care Agency clients with personal care needs.
- Review and follow current care plan developed for the client, and appropriately document care provided.
- Communicate with supervisor and Home Care Agency scheduler of availability to work specific days / hours, and provide timely requests for time off or other changes in availability.
- Attend departmental meetings and trainings, and comply with requirements for Relias Online Learning courses.
- Inform supervisor and nurses in the appropriate level of care of any changes or concerns with client functioning, health, and overall well-being.
