Wraparound Care Coordinator
The Wraparound Care Coordinator monitors, plans, links and advocates for their client and the client’s family. A Wraparound Care Coordinator should be available to help the family fold the needed services - including a full range of traditional and non-traditional mental health services, family supports, health, education, and social services - into a family-sensitive service plan.
Position Prerequisites
1. Bachelor's Degree from an accredited university or college, with a major in counseling, social work, psychology, criminal justice, nursing, rehabilitation, special education, health education, or a related human services field.
2. A minimum of one year of full time or equivalent experience working with children
3. Florida Driver’s License, ability to drive, and dependable transportation

1. Responsible to ensure all Phases and Activities of the Wraparound Process are provided to high fidelity
2. On-going collaboration and integration with all team members and education about wraparound
3. Education and support to promote self-reliance of the individual and family
4. Developing a strength-based assessment (Strength, Need and Culture Discovery) for each family served in accordance with state and federal criteria.
5. Developing an individualized Wraparound plan for each family served in accordance with all agency, contract, State, Federal and Medicaid guidelines, outcomes, and Wraparound requirements.
6. Working with the family to address needs related to implementation of their Wraparound plan.
7. Assessing the effectiveness of the Wraparound plan in meeting the identified needs of the recipient.
8. Linking the recipient with services and resources identified in the Wraparound plan,
9. Advocating for the acquisition of services and resources necessary to implement the Wraparound plan.
10. Coordinating the delivery of services as specified in the Wraparound plan in full partnership with the child, their family, system partners and their natural support system.
11. Monitoring service delivery to evaluate the family’s progress.
12. Documenting intensive Wraparound case management services in accordance with all agency, contract, State, Federal and Medicaid guidelines, outcomes, and Wraparound requirements.
13. Facilitating access to needed services documented in the Wraparound plan.
14. Immediate crisis stabilization.
15. Facilitate the Development of a Crisis plan and monitor the plan on a regular basis to ensure effectiveness.
16. Coordinate and facilitate monthly team meetings with the family, system partners and natural supports on a monthly basis to develop the Wraparound plan and to monitor the Wraparound plan to ensure the client’s goals are achieved and the plan maintains appropriate.
17. Monitor satisfaction with the Wraparound process.
18. Monitoring for transition readiness and Coordination/development of formal transition plan
19. Monitor weekly billing and submit a weekly billing report.
20. Responsible for ensuring compliance with all agency, contract, State, Federal and Medicaid guidelines, outcomes, and Wraparound requirements.
21. Perform any other duties as assigned by supervisor
Skills, Abilities, and Expectations
1. Knowledge of available resources in the service area for children with a serious emotional disturbance,
2. Knowledge of and compliance with state and federal statutes, rules, and policies that affect the target population.
3. A thorough knowledge of the stages, processes and milestones of normal physical, cognitive, social and emotional development of children from birth through adolescence.
4. Ability to learn and retain a comprehensive knowledge of agency policies and procedures.
5. Understanding of how children's behavior needs may be symptoms of underlying development delays or emotional disturbances.
6. Ability to educate and advise families and caregivers on age-appropriate expectations for children and can help set realistic expectations for children who demonstrate developmental problems as a result of a serious emotional disturbance or abuse and/or neglect.
7. Knowledge of how social work values and principles apply to child welfare practice, including respecting the family's dignity, individuality, culture and right to self-determination.
8. Understanding of the importance of effective case assessment and planning as the foundation of casework intervention.
9. Knowledge of the proper sequence of steps in the case planning process.
10. Comprehension of the factors that must be addressed in a comprehensive and balanced family assessment, including the contributing factors to abuse or neglect; the functioning of the family as a unit; the cognitive, behavioral, social and emotional strengths and limitations of each family member; and resources available to the family.
11. Knowledge of strategies to engage family members and kinship members into constructive and collaborative casework relationships that empower families and promote joint case assessment, planning and service provision.
12. Understanding of the dynamics of resistance and knows how casework methods can defuse family member's hostility, fear and anger.
13. Knowledge of how to integrate casework methods with authority, when necessary, to simultaneously engage and empower family and kinship members.
14. Understanding of the potential effects of cultural differences on the development of the client/caseworker relationship and knows strategies to establish relationships with family and kinship members from cultural backgrounds different from his/her own.
15. Knowledge of how to involve family and kinship members in the development of appropriate, time-limited case goals and objectives; how to formulate observable, behavioral measures of these goals and objectives; and how to collaborate with family members to identify the most appropriate services and activities to achieve service plan objectives.
16. Knowledge of the criteria by which one prioritizes service needs and objectives and can work with families to design service plans that address high priority needs.
17. Ability to promote delivery of effective services to families through case management, referral to community resources, utilization of community support systems, including non-traditional and neighborhood resources, utilization and expansion of family support, and the direct provision of supportive and rehabilitative services.
18. Knowledge of strategies to conduct effective interviews. This includes communicating the purpose of the interview; controlling the process and direction of the interview while encouraging family members to participate; using a variety of interviewing methods, including open and closed-ended questions, clarification, support, summarization and confrontation; helping families communicate feelings as well as facts.
19. Knowledge of how family-centered casework methods are used to promote family strengthening and stability for children by involving parents, kinship members and appropriate community members in assessment/ service planning process; providing services to maintain children in their own homes; assuring family members' involvement with their children; and providing the necessary services to achieve timely outcomes for children.
20. Understanding of the importance of conducting routine and timely service plan reviews with families and kinship members knows how to reassess the outcomes of all service plans and service interventions and make appropriate plan modifications. Understands the importance of reviewing each child's progress towards outcomes at each review.
21. Ability to plan, organize and manage the workload, set priorities, and perform activities in a manner that makes the best use of existing resources and time.
22. Understanding of the origin and consequences of work-related stress and has well-developed coping and management skills to prevent burnout.
23. Knowledge of how to recognize indicators of potential danger and knows strategies to reduce risk of personal harm or injury when making home visits or interviewing hostile or violent clients.
24. Ability to be available by cell phone to their assigned clients and supervisor
