Job Description: The Family Support Case Manager will provide direct services to families enrolled in the Families UPward program. Responsibilities include conducting initial assessments, developing individualized service plans, providing crisis intervention and ongoing support, making referrals to community resources, and advocating for families within the system. The Case Manager will work collaboratively with a team of professionals to ensure the well-being and safety of the families served.
