What are the responsibilities and job description for the APP-Primary Care position at UBMD Internal Medicine?
UBMD Internal Medicine is looking to add a Full Time NP or PA to our Primary Care Team!
Position Summary:
The Advanced Practice Provider (APP) will initiate the evaluation of patients with acute problems, perform specified minor procedures and participate in certain other activities of the Department as assigned by the Medical Director and/or Collaborating Physician.
Essential Functions:
Ø Examines patients to obtain information about their physical condition.
Ø Initiates the evaluation of patients with acute problems, gathering appropriate data through history, ordering appropriate tests (i.e. EKG’s, chest x-rays, laboratory tests, etc.). Following appropriate data collection and evaluation, he/she will relay findings to the patient’s attending physician and decisions will be made regarding the care of the patient.
Ø Instructs and counsels patients about prescribed therapeutic regimens, normal growth and development, family planning, emotional problems of daily living, and health maintenance.
Ø Performs certain other assignments as determined by the Medical Director and/or Division Chief including, but not limited to, participation in departmental conferences; aiding appropriate to level of training, and participation in the Quality Assurance efforts of the department.
Ø Functions as a member of clinical team in maintaining patient flow and maximizing the delivery of quality care to patients.
Ø Emphasizes health promotion and/or disease prevention through patient education and coordination of care.
Ø Reviews and addresses all assigned items related to documentation, tasking and results within the EHR.
Ø Completes all documentation and billing related to encounters no later than 5 business days from the date of service.
Ø Assists with tracking or documenting any quality measures relating to identified payers or programs the practice is enrolled in.
Ø Responsible for reviewing company email daily.
Ø Discusses concerns with Collaborating Physician, Medical Director and/or Division Chief in a timely manner to ensure efficient and effective patient care.
Ø Follows regulations and guidelines for writing prescriptions.
Minimum Job Requirements:
Education: Current NYS License as Physician Assistant or Nurse practitioner.
Experience: Minimum two years’ experience with healthcare delivery, preferably in primary care and/or family medicine. We are willing to consider newer graduates with a focus in Primary Care.
Knowledge, Skills
& Abilities: Excellent communication and attention-to-detail skills required. Experience with electronic health records required.

Pay: $90,000.00 - $105,000.00 per year
