Position Title: Intake Specialist II
Department: Teen Parenting Service Network (TPSN)
Reports to: Supervisor of Intake
Position Summary:
The Intake Specialist II uses their project management skills to coordinate and implement various administrative functions and projects. In addition, the intake specialist serves as a liaison between our System Support Services staff, various DCFS departments and child welfare service providers for the purpose of collecting, disseminating and tracking youth information and youth/worker resources essential for the provision of quality services.
Essential Duties and Responsibilities:
Intake Screening and Data Processing
• Coordinate and complete the screening process for new youth entries:
Diligently pursue communication with service providers in order to gather the information necessary to complete a thorough screening and ensure that all relevant information is captured.
• Enter information into corresponding Evolv- Parent in Care Enrollment screens.
• Communicate and Coordinate with System Support Services, DCFS and Service Providers as needed to ensure data updates, youth transfers and resources are managed and monitored effectively.
Advocacy
• Act as liaison between System Support Services, DCFS and Services Providers to ensure essential data is captured, linkage to appropriate resources are made and communication is ongoing.
• Provide individual support/assistance to Service Providers to ensure Intake process and procedure is followed effectively.
• Coordinate with System Support Team members to identify barriers and problem areas in order resolve and troubleshoot problems.
Quality Assurance and Reporting
• Monitor information system accuracy by running assigned discrepancy reports and making corrections to data as needed.
• Conduct data updates with Service Providers semi annually and as needed.
• Maintain youth information and updates as needed.
• Coordinate with System Support Services in order to procure and update youth information.
• Coordinate with Quality Improvement to establish consistent expectations for documentation, transfer protocol, and quality assurance.
Resources, Training & Communication
• Coordinate with System Support Services staff to stay abreast of youth/worker resources to communicate accurate and timely information to our providers.
• Coordinate with System Support Services staff to identify gaps and barriers in youth/worker resources in order to ensure appropriate solutions are implemented.
• Coordinate with System Support Services staff in order to identify training needs, publicize trainings and training resources.
• Additional tasks as assigned.

• Develop leadership skills through participation in trainings, committees, focus groups, readings, and seminars.
• Build collaborative relationships with associates and service providers to facilitate program improvements.
• Model solution focused problem-solving skills and work to empower and lead associates to do the same.
• Coordinate and implement various administrative functions and data management projects as needed to improve and enhance youth/worker services and programming.
• Develop and implement strategies (i.e. training, presentations, communications) to facilitate program improvements.
Education /Job Experience/Certification:
• BSW or bachelor’s degree in human services or related field required
• At least four years experience in human services
• Minimum one year as TPSN Intake Specialist I
Special Knowledge and Qualifications:
• Demonstrated ability and expertise in Project Management
• Demonstrated Leadership ability
• Familiarity and experience with computers
• Excellent writing, communication, advocacy, and presentation skills
• Strong sense of responsibility and orientation to detail necessary
• Automobile and valid Illinois driver’s license
