Position Objective and Summary:
UCAN is seeking a highly organized and compassionate individual to serve as the Mobile Crisis Response (MCR) Intake Coordinator. This role involves managing the intake process for individuals seeking crisis intervention services, ensuring that clients receive timely and appropriate care. The Intake Coordinator will be responsible for initial assessments, coordination of services, and maintaining accurate documentation.
Essential Functions and Responsibilities:
Intake and Assessment
- Conduct initial phone screenings and assessments for individuals seeking crisis intervention services.
- Determine the level of urgency and appropriate response for each case.
- Gather and document relevant information, including medical history, mental health status, and current crisis situation.
Service Coordination
- Coordinate the deployment of the Mobile Crisis Response Team based on the needs identified during intake.
- Collaborate with team members and other service providers to ensure seamless and comprehensive care.
- Follow up with clients and team members to ensure that services are delivered effectively.
Documentation and Reporting
- Maintain accurate and up-to-date records of all intake assessments, client interactions, and service coordination activities.
- Prepare and submit reports to the Mobile Crisis Response Supervisor as required.
- Assist in data collection and analysis to monitor program effectiveness and identify areas for improvement.
Client Support
- Provide clients with information about available services and resources.
- Offer crisis intervention support and guidance over the phone as needed.
- Ensure that clients feel heard, respected, and supported throughout the intake process.
Collaboration and Communication
- Work closely with the Mobile Crisis Response Team, other UCAN programs, and community partners to coordinate care.
- Communicate effectively with clients, families, and service providers to facilitate access to necessary resources.
- Participate in team meetings and case conferences to discuss client needs and service coordination.
Education/Job Experience/Certification:
- Bachelor’s degree in Social Work, Psychology, Counseling, or a related field required. Master’s degree preferred.
- Minimum of 2 years of experience in crisis intervention, mental health services, or a related field.
- Previous experience in intake coordination or case management preferred.
