The Residential Intake Coordinator will have primary responsibility for the intake process for admissions into our Residential programs
Primary Responsibilities:
- Manage the admissions process for individuals receiving supports and services including completing and maintaining all intake documentation. This will require understanding each program and necessary admission requirements
- Primary contact for referrals through OPWDD, Care Managers and families
- Review various OPWDD Needs lists for suitable individuals; gather and review all documentation and forward to intake committee
- Liaison with Care Managers and Regional Office for obtaining eligibility
- Ensure proper follow up with all intake committee members to make timely decisions
- Participate/coordinate trial visits and pre-placement meetings for all new admissions
- Develop and maintain an internal system for monitoring enrollment progress. Give reports as necessary
- Ensure changes to individuals service authorizations and amendments are submitted to OPWDD
- Gather necessary documents for all new admissions
- Coordinate all Internal Movement
- Ensuring all Medicaid and waiver documentation is in place prior to admission
Qualifications
- Bachelor's Degree preferred; High School Diploma
- At least 2 years working with individuals with intellectual disabilities
- Complete all state and agency required training
- Excellent verbal, written, interpersonal and communication skills
- Knowledge and understanding of local and regional regulatory operations
- Knowledge and understanding of OPWDD regulations
- Understanding and commitment to community-based support for persons with disabilities
- Experience with intake and admissions
- Excellent time management skills
- Proficient in Microsoft Office (Outlook, Word, and Excel)
