Job Summary
UCSF is developing innovations to support population health as part of California’s Quality Incentive Pool Program (QIP). In order to succeed in these new arenas, we are developing new partnerships and care delivery models, which need strong project management, leadership and support. The programs developed in population health help achieve the “triple aim” of high value care: they provide higher quality, lower cost, and patient centered care across the full care continuum. Examples of the type of work in need of project management include leading the implementation of new patient engagement and health equity strategies focused on preventive care, chronic disease management, and community partnerships.
Given the type of work and our organizational culture, the Project Manager will need to be able to collect and interpret data from various sources, have strong problem solving and presentation skills, have a good understanding of clinical care and be a strong team player. They will serve as the liaison to external healthcare partners as well as key UCSF physician leaders and clinic directors.
Given the early stage of this area of work, the Project Manager must be capable of developing new, feasible ideas to improve health and reduce costs for these targeted populations and able to take on new projects as this area of work continues to rapidly evolve. The Project Manager will provide project management support for division-wide initiatives and dissemination of Population Health innovative care models, as needed.
This Project Manager role is part of a dynamic Quality & Performance Improvement team within the Office of Population Health (OPH). This position will report to the Manager, Medi-Cal Quality and works in close collaboration with the leadership of the Office of Population Health.
The Project Manager is a flexible role that requires 10% on-site at variable UCSF locations in San Francisco and 90% remote.
The incumbent should be able to provide a safe and private home office environment for conducting patient outreach and UCSF business.

OPH is responsible for fulfilling UCSF Health’s accountable care contracts and fostering care partnerships across a clinical network.
This department is comprised of four main teams: (1) Clinical Programs, (2) Population Health Quality and Performance Improvement, 3) Ambulatory Risk Capture/Medicare Advantage Operations, and 4) Population Health Administration.

Required Qualifications
- Bachelor's degree in healthcare or a relevant profession, or equivalent experience/training.
- Project management certification from a reputable organization such as PMP, CSM or ACP to include a qualifying exam and / or quality assurance processes like Six Sigma or Lean or equivalent project management experience required.
- Two (2) years of recent experience in health system administration, ambulatory, primary, or specialty care.
- Requires advanced knowledge of project management principles and performance improvement concepts.
- In-depth knowledge of population health, NCQA, CMS, and/or HEDIS quality standards, and accountable care or value-based care principles. Broad knowledge of initiatives and an understanding of impact on the organization of such initiatives.
- In-depth knowledge of medical terminology and patient health information.
- Experience analyzing complex projects with broad impact. Ability to manage project progress within time and budget constraints.
- Highly proficient in multiple software applications (Word, Excel, PowerPoint, Access, Trello) and database applications. High level of proficiency in Microsoft Excel with the ability to run analytical reports and summarize information.
- Demonstrated capacity to anticipate location-specific obstacles and plan for contingencies.
- Demonstrated attention to detail. Demonstrated ability to determine relevance and prioritize tasks.
- Demonstrated advanced organization and project management skills, with the ability to lead a team, prioritize tasks, and see projects through from inception to completion on schedule.
- Demonstrated advanced effective writing skills. Advanced knowledge of all aspects of communications. Demonstrated ability to effectively communicate with all levels of an organization.
- Demonstrated competency in selecting and implementing project management methods and techniques.
- Proven leadership ability. Experience leading a team of professionals. Advanced interpersonal skills for collaborating effectively with a broad range of clinical and professional staff within a complex organizational environment.
- Advanced critical thinking and problem-solving skills to manage highly complex information, assess problems, and develop effective solutions.
- Advanced written and verbal communication skills with the ability to convey complex information in a clear, concise manner and produce high-quality documents, reports and presentations.
- Ability to exercise professional judgment in handling sensitive and confidential issues.
- Master's Degree in health care administration field, nursing or other health field and a minimum of 2 years recent experience in health system administration, ambulatory, primary or specialty care
