Key Responsibilities:

• Case Management Expertise: As an RN Case Manager, you’ll be assessing the psychosocial, medical, and discharge needs of patients and developing care plans that help drive their recovery journey.
• Collaborative Care: Work closely with social workers, physicians, and other healthcare team members to ensure the best outcomes for patients.
• Discharge Planning: Coordinate and ensure smooth transitions for patients as they leave the hospital, ensuring they are prepared with the necessary resources and follow-up.
• InterQual Utilization: Evaluate patient care progress using InterQual criteria to ensure quality and appropriate care throughout their stay.
• Emotional & Social Support: As part of the multidisciplinary team, you’ll also be involved in helping patients and their families navigate the emotional and social aspects of their care.

Qualifications:

• RN Licensure in Texas (don’t leave home without it!).
• Case Management Experience: Minimum 2 years in Acute Case Management.
• BLS Certification required (because patient safety comes first).
• Preferred: Social Workers with 1-2 years of Acute Social Worker experience (licensed in Texas) and MSW.
