Responsibilities

Seeking a full-time Clinical Assessment Coordinator to work on the inpatient Behavioral Health Unit in the Methodist Jennie Edmundson Hospital in Franklin, IN. 

The Clinical Assessment Coordinator, in conjunction with the Program Director, coordinates referral development and pre-admission patient assessment process, and provides clinical assessments for potential clients.

 

  • In coordination with the Director, develops, implements, coordinates, and evaluates clinical admission, referral, and/or discharge processes.
  •  
  • Responds to intake calls and completes admission paperwork. Conducts on and off-site patient assessments, including appropriate test administration, interpretation, and recommendations. Utilizes assessment tools including, but not limited to Geriatric Depression Scale, Zung, SLUMS, alcohol screening tool(s), etc. Coordinates and participates as a direct liaison with appropriate interested parties and physicians through the patient referral inquiry, admission, treatment, and discharge process. Addresses issues in a timely manner including crisis and higher level of care referrals. Reviews newly admitted patient accounts for eligibility and authorization daily.

  • Participates in the development of treatment plans in coordination with the treatment team. Coordinates treatment with physicians and other health and social service agencies. Facilitates
  • groups, family, and individual sessions and completes all necessary documentation. Provides appropriate support to patients’ families.

  • Enters required data into HorizonPlus system and generates monthly reports to track and trend referral, admission, and discharge data. Utilizes data to complete Program’s key statistical
  • reports and generates reports as needed for Horizon Health’s S/VPO, VPCE, Director and JMH (Johnson Memorial Health) Leadership when requested.

  •  Actively participates in department processes as requested. Attends unit and JMH meetings asdirected.
  •  
  • Responds to requests for general information about the Program’s treatment program, andprovides information about appropriate topics in response to community requests. Informs
  • Director, Horizon Health’s S/VPO and VPCE and JMH Leadership of any potential adverse publicity. Appropriately represents the Program at public and private outreach ev
  •  
  • Assists in submission of reports and requests for documents from appropriate regulatoryagencies. Prepares and disseminates monthly reports and completes required follow up onactivities on behalf of the Director. Prepares and submits all operational tasks/documentation(census report, PIORS) as required by Horizon Health’s corporate offices or JMH Leadership.

  • Regularly collects data to provide input and/or feedback to appropriate customers/suppliers (i.e., teams, supervisors, co-workers, vendors, nursing agencies) to improve work. Follows CQI  procedures.
  •  
  • Promotes and develops positive, professional interaction with patients and their families, peers, leadership, clients, etc. Corrects issues identified and reports errors to the Director.

  • Maintains appropriate ethical and clinical boundaries with patients, family members and other staff members. Maintains compliance with state and federal confidentiality laws as well as upholding JMH’s and Horizon’s confidentiality policies and procedures. Maintains a professional code of conduct as established by Horizon’s Code of Conduct.  Maintains schedule for filing court paperwork and hearings. Works closely with court personnel to maintain good relationships.

  • Protects patients and employees by adhering to safety standards, infection-control policies and protocols. As needed, and using approved methods, assists in safely placing patients in seclusion or restraints; observes suicide/seclusion precautions; including physical requirements for physical holds and safe transport of patients.

Qualifications

Requires a BSN and current Indiana RN license or master’s degree in social work, counseling or related field and current Indiana license as a LMHC, LCSW, or LSW. Current unrestricted Indiana Driver’s License and adequate personal automobile insurance required. Must have reliable personal transportation. At least 3 years of clinical experience in behavioral health setting.
