Via Linda Behavioral Hospital is a brand-new behavioral health provider serving Scottsdale and the great Phoenix region. We opened in February 2022 and now offer a full continuum of inpatient and outpatient services. Under the direction and supervision of the Director of Clinical Services, the Discharge Planner provides critical support and care coordination the patients of Via Linda Behavioral Hospital. The Discharge Planner is responsible for assisting and coordinating with the Clinical Services team to assure that patients have excellent and safe discharge planning, as well as providing recommendations for post-discharge support.
Qualifications
Clinical Skills
- Provide quality assessments and reports according to training and areas of expertise
- Provide quality individual, group, marital, and family education, according to training, specialties, and effectively manage time in clinical sessions
- Provide growth promoting experiences to patients
- Develop initial rapport with patients
- Define clear and meaningful treatment goals for patients
- Facilitate therapeutic movement toward treatment goals at an appropriate pace
- Demonstrate use of various and active strategies in interpersonal skills
- Help develop and evaluate patient-specific treatment plans
- Ensure treatment follows goals outlined in the treatment plan
- Provide or arrange proper family education.
- Develop and implement appropriate aftercare and discharge plans
- Complete all discharge forms, contracts, and crisis prevention plans with the patient and ensure that aftercare appointments are made prior to discharge.
- Contact aftercare professionals/treatment facilities to provide patient information that will aide in a smooth transition of care.

House Unit Clerk (HUC) Job Requirements:
- Must be able to obtain a Level 1 Fingerprint Clearance card.
- High school diploma or GED equivalent.
- Preferred one year of experience working with psychiatric patient population.
