Position Summary
This position is responsible for providing and coordinating comprehensive infusion services in a Home Care environment using nursing processes and standards of care. This position works cooperatively with pharmacy and team members to establish priorities and goals of patient care while maintaining standards of professional nursing practice in a Home Care setting.
Position Responsibilities
- Administer IV therapies to home care patients based on orders from physicians and other medical professionals.
- Admit patients to the University of Iowa HomeCare (UICH) services, handling all aspects of the admittance process.
- Perform patient visits according to the Plan of Care or as patient need dictates.
- Complete required patient care documentation, nursing plan of care, and reports.
- Coordinates the referral process for infusion therapy services with pharmacy and outside referral sources.
- Communicates regularly with the pharmacist regarding the specific instructions and needs of UICH patients.
- Perform case management activities for assigned patients.
- Participates in the development, implementation, and evaluation of home infusion therapy education protocols and materials.
- Educates patients and families on therapies and services provided.
- Actively participates in UIHC quality assurance programs.
- Participates in nursing staff education and training programs and maintains skills for assessing, delivering, and teaching home infusion therapy patients according to UICH Policies and Procedures and industry standards.
- Responds to verbal and written communications, concerns, or complaints from home care patients, families, care teams, insurance companies, and contract vendors.
- Participates in on-call duties as defined by the on-call program.
- Perform other duties as assigned.
