UI Community HomeCare is seeking a Intake Coordinator Infusion to fill a full-time position at our Iowa City office.
This is a Monday- Friday, day-shift position with no weekends.

Position Summary
Coordinates infusion and enteral services for patients discharged from inpatient facilities and manages therapy changes for existing patients. Communicates regularly via telephone / email / instant messaging with social workers, UICH pharmacy, nursing, inventory, and warehouse staff, and insurance companies. Ability to multitask, attention to detail, and flexibility are important qualities for a candidate in this position.
Position Responsibilities
- Coordinates care for patients admitted to the UICH service
- Coordinates delivery method of drug with pharmacist
- Communicate regularly with insurance companies to verify coverage and benefits as well as obtain prior authorizations as needed
- Identifies agencies and coordinates provision of infusion and other needed nursing care with the agency
- Provides follow-up communication with nursing agencies, including but not limited to, drug infusion, IV access, and care issues
- Performs required documentation for initial care planning, inventory records, and verbal / written communication with referral sources
- Completes chart reviews (as needed) to ensure therapy meets insurance coverage criteria before accepting patients for services
- Coordinates changes to existing patient's therapy
- Communicates changes with patients, caregivers, UICH staff, physicians, and nursing agencies
- Modifies delivery tickets as needed according to new orders received
