Intake Coordinator

Position Overivew:
Referral Intake & Assessment:
- Handle inquiry calls regarding skilled home health services to assess intake appropriateness in consultation with an RN and authorization personnel.
- Verify available resources (staffing) before accepting referrals.
Payer Verification & Authorization:
- Collaborate with authorization personnel to confirm the acceptability of the current payer source.
- Run Medicare/Medicaid eligibility checks for all referrals.
Staffing Coordination:
- Ensure appropriate and qualified staff are assigned to authorized services based on reviewed orders.
- Work closely with RN Coordinators to identify potential issues before admission.
- Ensure diagnoses codes and intake paperwork are reviewed before sending field staff for admissions.
Admission Process Management:
- Accurately input demographic information and verify client details at the time of admission.
- Communicate key client information to caregivers before service initiation.
- Maintain accurate records of admissions, start-of-care (SOC), and staff assignments.
- Follow up with caregivers to ensure timely service initiation (within 48 hours) or document any delays.
Collaboration & Communication:
- Participate in clinical rounds and provide updates to the team.
- Stay informed on payer contract changes through regular communication with authorization and contract personnel.
- Foster effective communication with internal staff and external partners to ensure smooth service delivery.
- Engage in problem-solving and identify community resources within and outside the organization's scope.
Referral & Contract Management:
- Track "not admitted" referrals for contract renewal and service planning.
- Work with marketing personnel to address referral source challenges and identify trends.
Compliance & Confidentiality:
- Adhere to HIPAA regulations and maintain confidentiality in all job functions.
- Notify caregivers of resumed services for hospitalized clients and review required services.
Administrative & Support Functions:
- Provide clerical support to RN Care Coordinators as needed.
- Participate in community development and marketing initiatives.
- Continuously update job knowledge through professional development opportunities.
Other Duties:
- Perform additional tasks as assigned to support the organization's mission.
