Your Impact
- Serve as a primary point of contact for patients, caregivers, and healthcare providers to facilitate smooth transitions of care
- Conduct initial assessments to identify patient needs, goals, and barriers to care
- Develop personalized care plans in collaboration with multidisciplinary healthcare teams and patients
- Coordinate appointments, referrals, and diagnostic tests to ensure timely access to healthcare services
- Advocate for patients to ensure they receive necessary treatments, medications, and supportive services
- Educate patients and caregivers about their health conditions, treatment plans, and self-management strategies
- Monitor patient progress and outcomes, adjusting care plans as needed to optimize health outcomes
- Document all patient interactions, care plans, and interventions accurately in electronic health records (EHR) systems
- Collaborate with insurance providers and community resources to address social determinants of health and facilitate access to supportive services
- Participate in care conferences and meetings to discuss patient progress, goals, and discharge planning
- Perform other duties as assigned by management needed to help drive to our Vision, fulfill our Mission, and abide by our Organization’s Values
- High school diploma or equivalent.
- One-year healthcare experience
- Certified Community Healthcare Worker (CHW) or become certified within first year of employment
- Proven experience in care coordination or patient navigation within a healthcare setting
- Strong understanding of medical terminology, healthcare processes, and patient care management
- Excellent interpersonal and communication skills, with the ability to interact effectively with patients, families, and healthcare professionals
- Organizational skills and attention to detail to manage multiple patient cases efficiently
