Responsibilities
- Conduct thorough patient assessments to identify needs and develop individualized care plans.
- Provide crisis intervention services to clients in urgent situations, ensuring their safety and well-being.
- Collaborate with healthcare professionals in hospital settings to coordinate care for patients.
- Offer specialized support for children with emotional or behavioral challenges, including those in special education programs.
- Deliver PTSD care and support to individuals affected by trauma.
- Maintain accurate medical records and documentation in compliance with HIPAA regulations.
- Collect and analyze data related to client progress and program effectiveness.
- Advocate for clients' rights and access to necessary resources within the community.
Requirements
- A degree in Social Work or a related field is required; licensure preferred.
- Experience working with children and families is highly desirable.
- Knowledge of crisis intervention techniques and patient assessment practices.
- Familiarity with ICD-9 coding for medical records is a plus.
- Strong communication skills, both verbal and written, for effective interaction with clients and professionals.
