SUMMARY OF POSITION:

The Aftercare Coordinator will provide on-going support and accountability through mentorship, support meetings and modeling a life of healthy recovery; cultivate healthy relationship with the LIFE Recovery Program residents with the goal of lifelong recovery and sobriety; and, develop, train, and supervise Aftercare volunteer support team. The Aftercare Coordinator will oversee UGM’s 14-unit Men’s Aftercare Housing and supervise the onsite Aftercare Housing Facilities Lead. 

 

DUTIES & RESPONSIBILITIES:

 

Key Result Area #1: MENTOR PROGRAM

  • Recruit, interview, and train mentors in order to provide 1 mentor for each Life Recovery Program participant, approximately 15-25 mentors per year. 
  • Ensure all Mentor’s understand and practice UGM’s cultural Core Values and their indicators. 
  • Ensure all mentors are trained in the UGM Mentor process and provided 4 mentor training opportunities per year.  
  • Follow up with all mentors monthly via email, phone or in person with goal of mentors meeting with mentee weekly.
  • Ensure Aftercare is collaborating with LIFE Recovery Program to promote successful transition and continued recovery of Program Graduates
  • Distribute monthly report of mentor participation to Recovery Program and Site Director.

 

Key Result Area #2: LEADERSHIP & MANAGEMENT

  • Attend specified treatment/program team and planning meetings to discuss the effectiveness of collaboration and programming components.
  • Develop, train, and supervise aftercare volunteer support team, meeting consistently to ensure continuity of care
  • Ensure Recovery Director is aware of concerns and issues regarding staff, residents, and program components
  • Collaborate with Aftercare Coordinators at each UGM recovery center to ensure consistency of Aftercare support
  • Supervise Men’s Aftercare Housing Facilities Lead.
  • Ensure staff know and participate in the Outcome Based Management process. 
  • Ensure accurate Aftercare data is entered into Waypoint Database on a weekly basis. 

 

Key Result Area #3: RECOVERY SUPPORT MEETINGS

  • Ensure the Recovery for Life 12-Step meetings are aligned with each site and that there is continuity within all LIFE Recovery Programs
  • Ensure the Recovery for Life 12-Step meeting is offered 1 time per week.
  • Encourage, educate, and develop volunteer leaders for the Recovery for Life 12-Step meeting, with a focus on LIFE Recovery graduates.
  • Provide leadership and resources for the Recovery for Life 12-Step meeting, directly managing and/or running meeting when necessary. 
  • Provide the LIFE Recovery Program team and Recovery Director with a monthly attendance record. 

 

Key Result Area #4: TRANSITION TEAMS/RELAPSE PREVENTION PLANS

  • Work with Staff and Program teams to develop individual Relapse Prevention and Transition Teams plans for each LIFE Recovery Program resident.
  • Train Recovery staff on implementation of Transition Teams
  • Participate in Phase 4 clients first Transition Team meeting and ensure clients have 3 meetings with Transition Team in Phase 5. 
  • Ensure monthly support contact with each program graduate via phone, text, or in person, for 3 years post program completion. 
  • Monitor and track graduate length of recovery for 3 years after graduation, with goal of each graduate reaching 5 years of sobriety. 

 

Key Result Area #5: TEACHING

  • Provide instruction in 12-step and recovery classes as requested.
  • Provide a Relapse Prevention Seminar to program participants once per trimester. 
  • Provide a Mentee Orientation once per phase 

 

Key Result Area #6: RESOURCE 

  • Build and maintain connections with outside support agencies including AA, NA and Celebrate Recovery, and other Treatment Facilities
  • Build and maintain connections with housing options for transition.

 

Key Result Area #7: MEN’S AFTERCARE HOUSING

  • Oversee UGM’s 14-unit Men’s Aftercare Facility and the onsite Aftercare Housing Facilities Lead. 
  • Prior to clients moving in, collect and ensure client’s Aftercare Housing application and fee agreement is complete and client’s understand housing expectations. 
  • Ensure Aftercare Housing Facilities Lead conducts a move in and move out inspection with client at time of move in and move out. 
  • Ensure collection of Aftercare Housing program fees prior to the 5th of every month. 
  • Hold residents accountable to guidelines set forth in Aftercare Housing Handbook.
  • In participation with the Men’s Program Team, determine appropriateness of applicants for Aftercare Housing.
  • In partnership with the Men’s Recovery Team and onsite building manager, conduct conflict resolution meetings with residents, as needed. 
  • Oversee and run weekly, onsite, support meeting at Men’s Aftercare Housing with goal of involving all program graduates for 3 years after program completion. 
  • Ensure a weekly bible study or small group is provided at Men’s Aftercare Housing and facilitated by a volunteer. 
  • In partnership with onsite Aftercare Housing Facilities Lead, assist residents and be the point of contact for concerns at the Aftercare Housing Facility. 
  • Oversee the Men’s Aftercare Housing budget, follow all credit card and budget procedures and ensure fiscal responsibility. 

 

SKILLS & QUALIFICATIONS:

Committed Christian, able and willing to model servant-leadership and interested in being part of a ministry for the poor, homeless, drug addicted and mentally ill. Grounded in the Christian faith as a mature, active member of the body of Christ. In accordance with Union Gospel Mission Statement of Purpose, Statement of Vision and Statement of Faith.

  • Strong Interpersonal skills - exercise compassion and discernment while modeling the teachings of Jesus Christ.
  • Able and willing, as a unified team member, to honor the leadership, chain of command, and standards and ministry objectives of Union Gospel Mission.
  • Computer literate (Office 365,Word, Excel, Outlook, Web-Browser, Waypoint, UKG Ready HR); able to complete documents, reports and accurately record information. Willing to continue education as needed or required.
  • Excellent communication skills:  written, verbal and instructional.
  • Must have a teachable spirit and the ability manage multiple emotional and task related variables, listen, and make sound decisions in alignment with UGM’s purpose. 
  • Minimum 5 years of sobriety, if in recovery.
  • Minimum of 3 years living outside of a UGM facility, if applicable. 
  • Insightful, trustworthy, honest, reliable, model appropriate boundaries, and strong work ethic.
  • Valid Washington or Idaho driver’s license– required. 
  • CPR/First Aid certification – required. 
  • Yearly TB clearance – required.
  • Criminal background check clearance – required.

 

WORKING CONDITIONS/PHYSICAL FACTORS:

Occasionally = 1%-33%; Frequently = 34%-66%; Continuously = 67%-100%

  • Work is primarily indoors and may require climbing three flights of stairs, sitting, standing, bending, reaching, and going from building to building - Continuously. 
  • Requires working with a diverse population, many of whom struggle with homelessness, addiction and mental illness – continuously.
  • Requires a sustained level of high energy, stamina, and the ability to move about all of the facilities on a regular and ongoing basis – frequently.
  • Requires the ability to hear, see, and speak clearly – continuously.
  • Requires weekly travel between facilities in Spokane and CDA—continuously. 
  • Requires late shift start and end times—continuously. 
  • Requires frequent meetings and/or phone calls with a person in relapse—frequently

 

EQUIPMENT/TOOLS USED:

  • Computer (Office 365, Word, Excel, Outlook, Web-Browser, Waypoint, UKG Ready HR), Copier/printer, Phone, Fax, Breathalyzer, and Drug Testing Kit.

 

EDUCATION/EXPERIENCE:

  • Recovery Coach, Peer Support, or related field – preferred.
  • 5 year’s prior related experience, or equivalent combination of education and experience.

 

REQUIREMENT:

Union Gospel Mission is a privately funded 501 (c)3 non-profit, evangelical Christian ministry.  Our designated purpose is religious and we are a Christ-centered ministry which is dedicated to sharing the Gospel and helping the homeless and impoverished.  We consider every position to be essential in the fulfillment of our ministry and Statement of Purpose.  As such, each employee must have a relationship with Jesus Christ as their personal Savior and Lord.  All employees must:

  • Be able and willing to share the Gospel and participate in all of the ministry activities of Union Gospel Mission;
  • Subscribe to Union Gospel Mission Statement of Faith and Statement of Purpose upon hire and continuously while employed;
  • Adhere to Union Gospel Mission Employee Handbook.  

Men’s Aftercare Manager Expectations List

General –

  • Demonstrate consistent application of the Union Gospel Mission’s Core Values in daily interactions and decision-making.
  • Ensure that your assigned work is completed on time and meets the specified quality standards.
  • Ask for help when needed.
  • Demonstrate consistent application of employee and department policies and procedures.

Daily –

  • Review Shift Log Report from previous day and follow up on any concerns regarding Aftercare clients.
  • Check and respond to email and telephone communication.

Weekly –

  • Aftercare client files and Waypoint accounts are accurate and up to date. Waypoint Data Entry Expectations are followed.
  • Attend scheduled program team meetings to update staff on Aftercare clients, classes, and community meetings.
  • Ensure time clock entries are accurate and submitted by Monday morning after the pay period ends.
  • Ensure a minimum of 1 recovery support meeting is provided at Aftercare Housing and Men’s Mission.
  • Build and maintain connections with outside recovery support agencies including AA, NA, and Celebrate Recovery.

Monthly –

  • Collect Aftercare Housing fees and turn into Donation’s Processing by the 5th of each month.

Each Phase –

  • Biblical truth is fully integrated into the recovery process.
  • Consistently maintains confidentiality in accordance with established Program Guidelines and HIPAA.
  • Supports Genesis Process guidelines in the care of all residents, utilizing Genesis Process tools in counseling and classes.
  • Complete class critiques for assigned classes at the end of each phase and turn in to Program Director.
  • In partnership with AOH and CWC Aftercare Managers, conduct a minimum of 1 Mentor Training.
  • Recruit and train enough mentors so that every recovery program client has 1 mentor by the end of Phase 3.
  • Participate in Phase 4 clients first Transition Team meeting and ensure clients have 3 meetings with Transition Team in Phase 5.
  • Prior to clients moving into Aftercare Housing, collect client’s completed Aftercare Housing application and fee agreement and ensure client’s understand housing expectations.
  • Conduct move in and move out inspections for Aftercare Housing apartments.
