This role is responsible for engaging tenants at The Vineyards on E Lancaster in genuine relationships and facilitating community engagement and overall tenant well-being. Collaborate with property management to develop and execute community engagement activities.
Job Duties & Responsibilities:
- Host social activities like card games, dominos, movies, etc. for the purpose of community formation and socialization.
- Assist the Community Engagement Case Manager with Intake activities, warehouse vouchers, scan cards, and critical documents.
- Conduct daily welfare checks of tenants on a rotating basis. Maintain weekly visual contact with all homebound/disabled/elderly/unemployed tenants. Address home chore needs of elderly and disabled tenants.
- Ensure all new tenants and residents are seen at Healing Shepherd Clinic for the initial Healthcare Assessment within 3 days of Intake.
- As Spiritual Life issues arise, make appointments for tenants with the UGM-TC Chaplain. Assist the Chaplain with spiritual life activities, Bible Studies, and ALPHA participation; assist with volunteer provided events and activities, such as neighborhood crime watch meeting.
- Provide crisis management and intervention assistance when a situation warrants it. Report issue immediately to the Community Engagement Case Manager.
- Provide logistical guidance and assistance for new tenant move-ins and move-outs to reduce as much stress in the experience as possible.
- Enroll and update client information in the appropriate database system and regularly update. Input case notes in the appropriate information system within three (3) days from the date of an interaction or action taken.
- Use appropriate discretion and authority to enforce risk prevention and management practices and Policy and Procedure including but not limited to protected health information (PHI), client rights and incident management. Transmit PHI in strict adherence to the organization’s confidentiality and Health Insurance Portability and Accountability Act (HIPAA) Policy and Procedure.
- Positively contribute to building and maintaining team morale and actively contribute to creating an atmosphere of problem solving, learning, and open and honest feedback. Actively participate in individual and or group supervision and staff meetings.
- Complete annual training plan requirements. Independently seek out resources and learning opportunities to foster ongoing professional growth, including meetings and trainings coordinated and or facilitated by community partners.
- Support and execute on all Child Enrichment programs as instructed.
Required Skills & Knowledge
1. Have knowledge and experience in trauma-informed care & applying a trauma-informed approach.
2. Can easily build safe, trusting relationships with clients/guests and exercise appropriate boundaries.
3. Have knowledge of issues/traumas tenants face such as victimization, poverty, chronic health conditions, substance abuse/use, and intimate partner violence.
4. Can work independently with minimum supervision and maximum accountability.
5. Have excellent written and oral communication skills and excellent interpersonal skills.
6. Have proficient computer skills (Windows operating system and Office products, including Word, Excel, Teams, and Outlook) as well as an ability to learn and use any other software programs needed in the operations of UGM-TC.
