PCC Essential Job Duties:
- Scheduling and performing Medicare annual wellness visits for assigned providers.
- Assisting and scheduling patients to maintain up to date preventative services.
- Work with a multi-disciplinary team to manage care of chronic medical conditions.
Education, Training & Experience
- Must have active IN LPN license
- Must have graduated from an accredited nursing program
- Must have active BLS certification
