Job Summary:
The Coordinator of Day & Community Habilitation Services oversees the daily operations of these programs.
Essential Duties and Responsibilities:
· Oversee and monitor the daily operations of the Day Habilitation programs and services.
· Supervise the Day and Community Habilitation staff to ensure that all operations are occurring as per UCCS policies and procedures and OPWDD regulations/ State and Federal Law.
· Ensure the safety and well-being of all individuals enrolled in UCCS Day Habilitation services. Monitor and assess each consumer’s ability to self-preserve and their awareness of and ability to respond to harm or danger.
· Ensure that all service delivery, habilitative plans, daily and monthly service documentation, individual records, and billing documentation systems are maintained in compliance with all OPWDD regulations, State and Federal mandates as well as UCCS professional and person-centered standards.
· Provide ongoing supervision, training, and professional development to Direct Service Professionals. Ensure that all team members receive the appropriate training in accordance with OPWDD regulations as well as to promote their professional growth and development.
· Develop and maintain community partnerships to provide volunteer, social, recreational, and habilitative opportunities for all individuals served in the Day Habilitation programs
· Ensure that all services are provided in a therapeutic, safe, clean, and productive environment that promotes individual choice, independence and overall quality of life of individuals served.
· Oversee the transportation of individuals to and from the program, as well as community activities as needed.
Ensure that any issues that may impede the safe transport of individuals and/or staff are immediately reported to and addressed by the Director of Development.
· In conjunction with the Care Coordinator, individual, and/or parent, attend IDT meetings and Life plan reviews for all individuals served if necessary.
· Develop an individualized Habilitation plan using Person-Centered Approach.
· Develop productive and professional relationships/partnerships with service providers, DOE staff, community members, and OPWDD representatives to facilitate outreach efforts, marketing, and recruiting appropriate candidates to fill and sustain UCCS’ program vacancies.
· Ensure utilization of the Therap tracking system · Oversight of staff work schedules, personnel, and training records, documentation, payroll submissions, time and attendance tracking, petty cash, and professional staff development.
· Implement and monitor the DSP Core Competency model per OPWDD regulation.
· Monitor and track service units to ensure they are being maximized
· Work as a cohesive team member with other professionals
· All members of the UCCS’ management team are expected to be available as needed to address programmatic and/or staff emergencies.
· Attend conferences, in-services, professional forums, and informational meetings as assigned by the supervisor to enhance and ensure up-to-date knowledge of current mandates as well as continued professional development
· Maintain a Community Habilitation caseload · Any other duties as assigned by your supervisor.
Qualifications:
· A Bachelor’s degree in a related health or human services field
· A minimum of 2 years of experience in an OPWDD-regulated service area with at least 1 year of supervisory experience preferred
· Must have a working knowledge of the current regulations, policies, and protocols governing the delivery and documentation of services for individuals with Intellectual and Developmental Disabilities.
· Strong leadership, professional and organizational skills.
· Able to communicate departmental concerns and issues to supervisor in a timely manner
· Excellent communication skills (verbal and written)
· A New York State Driver’s license is preferred as travel is required between programs on at least a weekly basis
