BASIC FUNCTION:
Provide direct physical care and other supportive services to individuals in their home and in the community.
RESPONSIBILITIES:
1. Provide daily living skills training and supportive direct care services to clients as identified in their Individual Service Plan (ISP); for example, meal preparation, housekeeping, toileting, bathing, dressing, feeding, assisting with or administering medications, shopping, attending appointments, transporting, performing health care related activities, etc.
2. Ensure the safety and well-being of clients, including shoveling snow to clear walkways.
3. Honor the rights and confidentiality of clients, and treat them respectfully.
4. Provide input into the team regarding client needs, abilities, wants and desires.
5. Ensure client health/medical needs are reported and attended to on a timely basis.
6. Develop functional knowledge of DD, including terminology, and actively participate in skill training, programming, and maintaining physical, mental and emotional health of clients.
7. Perform routine and non-routine housekeeping duties to maintain a clean, safe and healthy living environment.
8. Report for work on time, as scheduled.
9. Attend and participate in staff meeting and training sessions. Attend supervisory meetings with manager on request.
10. Attend a minimum of 8 hours of training each year. Maintain all required training and certifications including, but not limited to CPR, First Aid, DD Med-Pass etc..
11. Complete incident reports and notify the appropriate person for unusual incidents.
12. Report any major unusual incidents to a supervisor immediately, and call the MUI hotline if there is any doubt the supervisor will perform that duty. Report any signs of abuse, neglect, injuries of unknown origin or questionable activity.
13. Collect data and complete required documentation BEFORE going "off-duty" and within established guidelines and standards.
14. Provide an adequate report of client needs and relevant information to other staff involved in client service activities.
15. Assist in developing a positive community awareness and acceptance of persons with disabilities.
16. Adhere to all Agency policies, procedures, and regulatory standards.
17. Other duties as assigned.
Qualifications
QUALIFICATIONS:
1. High school diploma or the equivalent.
2. Previous experience or educational background in the field of health and human services preferred.
3. Possesses a valid Ohio driver’s license with two (2) or less moving violations and has reliable daily transportation.
4. Sight and hearing sufficient to successfully complete the responsibilities of the job.
5. Writing and math skills sufficient to complete various documents.
6. Knowledge of and ability to lift, turn, position and transfer persons with physical disabilities in a safe and comfortable manner. Able to lift 50 pounds or more safely and operate a "Handi-Move".
7. Has reliable transportation available, when working, in order to transport self to work, meetings, and between sites;
8. Able to transport clients safely, using agency provided lift-equipped vehicles.
9. Able to operate common household appliances including stove, oven, microwave, dishwasher, iron, vacuum, clothes washer and dryer, etc.
10. Able to work pre-established hours, including weekends, based upon the needs of the program and clients.
11. Able to apply conflict resolution skills sufficiently to refrain from behavior or conduct deemed offensive or undesirable.
12. Courteous and cooperative manner.
13. Able to organize time efficiently.
