Role Overview:
The HCBS Care Coordinator is a critical role, responsible for supporting individuals with developmental disabilities by managing their care and coordinating access to community-based services while aligning with the regulations and expectations of the Arizona Department of Developmental Disabilities (DDD). As an HCBS Coordinator, you will be responsible for providing support to individuals who require assistance in managing their care and accessing community-based services. You will serve as a liaison between members, families, and service providers, ensuring that all care plans are properly implemented, monitored, and adjusted as needed.
This is a home-based role, but candidates must be willing to travel, have weekly in-office days and occasionally attend community outreach events.
Bi-lingual/Spanish preferred, but not required.
$2,000 Sign on Bonus offered.
Payable:
- $500 upon hire
- $500 after 90 days
- $500 after 180 days
- $500 at one year.
Key Responsibilities:
- Case Coordination: Manage case files and service plans for individuals with developmental disabilities, ensuring that needs are being met and services are delivered efficiently and effectively.
- Service Provider Liaison: Collaborate with DDD caseworkers, service providers, healthcare professionals, and families to advocate for and monitor the implementation of member care plans.
- Community Outreach: Attend community events, meetings, and job fairs to promote the services offered, network with local partners, and engage in outreach efforts to benefit our members.
- Member Assessments: Conduct in-home assessments to evaluate member needs, develop comprehensive care plans, and make proactive recommendations for services and supports.
- Documentation & Compliance: Maintain accurate, up-to-date records in compliance with all DDD regulations, ensuring confidentiality and timely reporting.
- Culturally Sensitive Care: Work with diverse populations, with a focus on delivering culturally appropriate services that meet the needs of the community.
- Innovative Problem-Solving: Bring creativity and forward-thinking to overcome challenges and ensure members receive the best care possible.
Qualifications:
Required Skills and Experience:
- Experience with Arizona Department of Developmental Disabilities (DDD): Familiarity with DDD services, policies, and regulations is essential.
- Case Coordination: Experience in case coordination and working directly with individuals with developmental disabilities.
- Member-Centered Approach: Demonstrated ability to work compassionately with individuals with developmental disabilities, and their families, in a proactive and innovative manner.
- Compassion & Commitment: A strong passion for serving others, with a commitment to improving the lives of individuals with developmental disabilities.
- Travel Required: Ability to travel to members' homes and attend community outreach events as needed.
- Driver’s License: Valid driver’s license with a clean driving record and current auto insurance.
- Must meet agency licensure requirements for fingerprinting, CPR and first aid.
- Technical Sills: Must be proficient in industry software. Also must be moderately proficient using Microsoft Word, Excel, Power point, and Outlook
Preferred Qualifications:
- Certified Case Manager (CCM): Certification is a plus, but not required.
- Bi-Lingual/ Spanish (both written and verbal)
- Associate’s Degree: A degree in healthcare, social services, or a related field is preferred.
- Experience in Community Outreach: Familiarity with community-based organizations and services is highly desirable.
