Job Duties: As a DSP, this role will provide coordination of medical aspects of assigned case load including transporting, documentation, and scheduling of appointments.
The purpose of the DSP-Health Liaison is to be the key point of contact between program individuals, and medical providers in addition to providing transportation, documentation and scheduling of appointments.
Certifications: Must have a valid GA Drivers License, CPR/FA and COR
Education Requirements (PREFERRED): Certified CNA or Certified Medical Assistant
ESSENTIAL JOB DUTIES ANDRESPONSIBILITIES:
Liaison between nursing staff and program staff regarding medical supports and need of specified caseload.
Transports individuals to appointments.
Schedules annual physicals, follow up appointments including dental, eye, adaptive equipment and any specialist.
Ensure all appointments are recorded in Therap. Send follow email to the appropriate parties which include Sr. CLM, RD, Nurse Manager, Director of Nursing and HRST Representative
Contacts nurse case manager prior to departure from appointments for nurse to be provided information.
Ensure refills of PRN and prescriptions are completed correctly and in a timely manner
Coordinate with Sr. CLM and Regional Directors the scheduling of medical visits; ensure the documentation required at visits is completed; collect and monitor all medical visit forms/documentation (review and distribute), including follow up data
Monitor the need for or the receipt of lab work; collect data and ensure proper follow up if required.
Positively, and effectively interact with regional boards, consumers, families and all levels of UCP staff.
Must be able to push, pull, lift 50 lbs or more with or without reasonable accommodations.
Other duties as assigned.
