JOB SUMMARY:   Provide operational and case management to assigned residential locations

 EDUCATION OR TRAINING REQUIREMENT:  Bachelor’s Degree in Social Service such as Psychology, Social Work, Sociology or related field. Must meet GA Developmental Disability Professional (DDP) requirements.

 EXPERIENCE REQUIREMENT: 5 years of related experience and 5 years of supervisory experience

SPECIAL LICENSES, REGISTRATION, CERTIFICATION, OR SKILLS:  GA Driver’s License and vehicle insurance; must be proficient with Microsoft Office Suite (Word, Excel, Power Point, Outlook, etc.)

SUPERVISORY RESPONSIBILITY: Supervises Community living managers

ESSENTIAL JOB DUTIES ANDRESPONSIBILITIES

Leadership  

 Ensures Area operates in compliance with local, state and federal rules and regulations and ensure all Community Living Arrangements (CLA’s) comply with Healthcare Facility Regulations (HFR) rules as well as Department of Behavioral Health and Developmental Disabilities (DBHDD) standards. Ensures residential locations operate in compliance with UCP policies and procedures.

  • Ensure staff are compliant with all personnel and training requirements, which includes reviewing weekly relias reports and handling all emergency situations
  •  Work Collaboratively with HR with identifying talent for Region and ensure appropriate hiring packets are submitted by established deadlines. 
  • Support managers in the development of their teams.
  • Schedule and facilitate monthly house meetings accomplishments.  Ensure staff is focused and well trained on ISP goals and personal outcome measures
  • Provide supervision to Community Living Managers (CLM’s)regularly. Conduct annual professional development and evaluations of CLM’s.
  • Coordinate activities with external agencies; Support Coordinators, DFCS, Social Security, doctor’s offices, community businesses and others, representing the individual you serve
  • Provides 1:1 training/support to identified staff with performance concerns and follow up as needed on progressive discipline.  
  • Approves status change and all other related HR forms related to processing of new employees 
  • Manages and creates on-call rotation 
  • Responds to all calls within 15 minutes unless not on call and responds to all emails within 24 hours.  
  • Support individuals in their personal growth and development, respecting cultural, ethnic, spiritual, and individual differences.

 Financial Responsibilities

  • Ensures Region remains in compliance with budgets (staffing, food, household supplies, etc). Conducts budget reviews on a quarterly/monthly basis.
  • Review for accuracy and submit monthly Petty cash and CTF reimbursement along with reconciliation form by due date to Director of Community Living.
  • Ensure individuals CTF’s do not exceed 2000.00 in any given month and authorize funds added to house credit card for individual purchases.

Responsibilities Relating to Health and Safety

  1. Submit completed site visit tool bi-monthly to Community Living Quality Coordinator which ensures that home meets all safety requirements and identify quality concerns
  2. Manages plans for potential emergencies and disasters, such as medical, fire, severe weather and missing persons. Discusses with Individual’s Support team specific role relating to each emergency.
  3. Approves GER’ and submits CIR’s and Safety Plans per UCP and DBHDD policies.
  4. Weekly review of Support Coordinator Ratings and coaching, referrals should be resolved in 15 days.
  5. Submission of Corrective Action Plans (CAP) when required. Responsible for ensuring all CAP related actions items are submitted by due date.

Case Management Responsibilities:

  • Runs and audits biweekly reports of all Therap documentation to include but is not limited to shift notes, ISP goal tracking and bowel tracking to ensure they are present and content is thorough and accurate.
  • Ensure transportation and supervision for residents as dictated by individual program.
  • Reviews the completed person-centered calendar each month which supports the implementation of activities as designed by the individual’s support team. 
  • Assess the needs of individuals and identify any required services, such as equipment, clothing, or other essentials.
  • Complete bi-monthly DDP Notes to ensure accurate documentation of individual progress and family contact is completed. Visit homes as noted in Therap Case Note and document the date and time of each visit
  • Conduct Nursing Assessments to evaluate and address the health and medical needs of individuals.
  • Participate in all SIS meetings
  • Participate in and complete all ISP (Individual Support Plan) Meetings, ensuring individual goals and objectives are met.

 

 

Other

  • Maintains objectivity in position to set appropriate limits while working with individuals.
  • Positively, and effectively interact with regional boards, individuals, families and all levels of UCP staff.
  • Review individual/family satisfaction surveys; implementing changes when feasible
  • Complete projects appropriately and timely as assigned by the CPO and DOCLS. 
  • Work cooperatively and responsively with all UCP departments and colleagues
  • Develop and maintain positive and productive relationships with individuals, families, co-workers, and community members.
  • Coordinator and serves as the facilitator for all residential admissions.  
  • Other duties as assigned.
