Key Responsibilities:
- Assess client needs to establish individual service plans.
- Act as a family support system and provide needed community resources for caregivers.
- Maintain client, family, and provider meeting and communication notes.
Requirements:
- Bachelor's Degree in social work - preferred
- Two (2) or more years' experience working with social service delivery to elderly population
- Working knowledge of resources available in geographic location serving the community
- Must be able to proficiently operate a computer, the Internet, Microsoft Office, other relevant software, and basic office equipment
