UCFS is hiring! We are seeking a Director of Clinical Quality Improvement to lead and oversee all aspects of clinical quality improvement initiatives, utilizing data-driven approaches to identify areas for improvement within clinical practice, design and implement interventions, monitor outcomes, and ensure that UCFS consistently delivers high-quality patient care while adhering to regulatory standards and best practices, often collaborating closely with providers, staff, and leadership to achieve optimal patient outcomes across the health center network. This position is evenly split between providing direct medical care at an outpatient health center and working on clinical quality improvement initiatives.
General Duties and Responsibilities:
- Collaborate with the CMO and Director of Population Health to analyze clinical data, identify areas for improvement across the primary care clinical department, establish clear metrics and targets for quality improvement initiatives and prepare for value-based care. Develop and implement comprehensive quality improvement plans aligned with UCFS goals and national quality measures.
- Provide quality medical care to clients/families; ensure patient communication and documentation with clinical findings, recommendations and treatment plans while promoting healthy behaviors and lifestyles.
Primary Care Services
- Responsible for providing quality patient centered primary care services for patients including comprehensive physical examinations, routine and appropriate sick care assessments and interventions.
- Develop and implement patient management plans, records, and progress notes per agency policies and procedures.
- Perform therapeutic procedures such as but not limited to; injections, immunizations, suturing, wound care, instruct and counsel patient and families regarding adherence with therapeutic regimen, normal growth and development and health promotion recommendations.
- Coordinate care with other providers of care and fosters patient advocacy by interfacing with community resources. Establish and maintain patient continuity of care.
Clinical Practice Transformation:
- Facilitate the adoption of evidence-based clinical guidelines and best practices within UCFS. Monitor adherence to clinical guidelines through chart review or other methods.
- Collaborate with primary care providers to design and implement practice changes aimed at improving patient outcomes.
Data Analysis and Reporting: Work closely with the Director of Population Health, the Medical Information Analyst Lead and the Healthcare Informatics Manager to:
- Gather, analyze, and interpret clinical data from electronic health records (EHR) and other sources.
- Generate comprehensive reports on quality performance metrics to inform decision-making at all levels of the organization.
- Utilize data visualization tools to effectively communicate quality trends and progress to staff at all levels.
Staff Education and Training:
- Provide ongoing training to clinical staff on quality improvement methodologies, including Plan-Do-Study-Act (PDSA) cycles.
- Empower clinical teams to participate in quality improvement projects and champion quality initiatives.
Regulatory and Grant Compliance:
- Monitor compliance with all relevant healthcare regulations, accreditation standards, and grant deliverables related to quality improvement.
- Develop and implement strategies to address any identified compliance gaps.
Stakeholder Management:
- Collaborate with UCFS leadership, medical directors, and other stakeholders to align quality improvement goals with organizational priorities.
- Effectively communicate status of quality improvement initiatives and outcomes to internal and external stakeholders.
Requirements
- MS in Nursing or Graduate of an accredited Physician Assistant program
- At least 5 years of experience in primary care medicine
- Prior experience with community health care preferred
Certificates, Licenses, Registrations
- Licensed to practice in the State of Connecticut
Other Skills and Abilities
- Strong knowledge of quality improvement methodologies and tools
- Excellent leadership, communication and interpersonal skills
- Ability to work collaboratively with clinical staff and other stakeholders and demonstrated ability to work positively with patients
- Knowledge of regulatory standards, current trends and best practices in primary care medicine
- Proficient in Epic EMR with strong analytical and problem-solving skills
