We are seeking for a highly qualified Quality Assurance Coordinator. As a Quality Assurance Coordinator, you will be primary responsible for defining the Quality Improvement program to Agency personnel and facilities compliance with the Agency standards, regulatory bodies, and standards of practice. Our ideal candidate is an efficient, dynamic, and cooperative individual who can perform well while juggling multiple tasks with little to no direct supervision.
ESSENTIAL DUTIES AND RESPONSIBILITIES
· Implements and monitors the Agency’s Quality Improvement Program.
·Implements processes to monitor and evaluate safety, risk management, infection control, and competency programs.
· Works collaboratively with clinical managers to assure documentation is complete, accurate, reflects care delivery and legal requirements.
· Coordinates the Agency's clinical record review activities.
· Monitors utilization of services, client concerns, and risk management programs. Works with other management staff to follow up on expressed concerns of clients, families, employees, referral sources, and physicians. Attends client care conferences and monitors progress and documentation of activities and outcomes.
· Makes home visits as needed to observe care delivery and client satisfaction. Assesses utilization of supplies and equipment.
· Performs job in compliance with Agency policies and procedures and professional and community standards. Provides services in accordance with applicable state, federal, CHAP, and JCAHO standards.
QUALIFICATIONS
· Bachelorette or Associates Degree.
· Two years hospice related experience and/or training; or equivalent combination of education and experience.
· Knowledge of medical terminology.
· Knowledge of the regulatory requirements for home care, including the HIS data collection and reporting requirements and OBQI expectations for Medicare Certified Agencies.
· RN/LVN Preferred
· Home Health and Hospice Experience
