Job Overview
We are seeking a dedicated and compassionate Registered Nurse (RN) to join our family owned growing Home Health team. In this role, you will provide high-quality nursing care to patients in their homes, ensuring their comfort and well-being. You will be responsible for developing individualized care plans, managing patient records, and supervising home health aides. The ideal candidate will possess strong leadership skills and a thorough understanding of acute care practices.
Responsibilities
- Assess and evaluate patients' health status and needs in a home environment.
- Develop, implement, and update individualized care plans based on patient assessments.
- Supervise and mentor home health aides to ensure quality care delivery.
- Maintain accurate medical records and documentation in compliance with Medicare guidelines.
- Provide education to patients and their families regarding health management and care plans.
- Collaborate with interdisciplinary teams to coordinate patient care effectively.
- Monitor patients’ progress and adjust care plans as necessary to achieve optimal outcomes.
- Ensure adherence to safety protocols and best practices in home health settings.
Experience
- Current RN license with a Experience in Nursing, preferably in home health or acute care settings.
- Proven experience in supervising staff and managing shifts effectively.
- Strong knowledge of physiology, medical records management, and Medicare regulations.
- Excellent leadership skills with the ability to motivate and guide team members.
- Familiarity with developing care plans tailored to individual patient needs is essential.
