Case Manager Position
Position: Non-exempt, 40 hours weekly reporting directly to the Program Director.

Work Hours: 8 am – 4:30 pm, Monday-Friday. NOTE: Hours may be flexible based on tenant needs.

United Housing Connections (UHC) is where compassion meets action. Founded in 1996, the mission to connect people at-risk for or currently experiencing homelessness with safe, sustainable, and affordable homes. We work to promote and preserve the dignity and quality of life for citizens in the Upstate of South Carolina. UHC works to meet the needs of vulnerable populations by providing holistic individual case management with a goal of offering a path toward self-sufficiency and independence.

The UHC case manager is responsible for obtaining housing, providing support services, and connecting program participants to community resources to help them become stably housed and as self-sufficient as possible. The UHC case manager is also required to maintain regular contacts with each participant based on their service plan, helping them to develop a life plan to overcome the obstacles, where possible, that prevents program participants from becoming stably housed on their own.

This position requires an energetic professional with exceptional people and communications skills. The case manager should be willing to make connections in the community with service providers to help participants receive the assistance needed. In addition, keeping up-to-date case notes and files on every client to track participant progress is essential. The ability to be self-motivated and work independently is a must. Company vehicles are offered.

Responsibilities:

  • Locate available units and support the lease up process based on the needs of the program and the clients being served.
  • Follow housing-first guidelines, while strongly encouraging program participants to take advantage of every resource available to help move them toward independence.
  • Obtain the verification documentation needed for eligibility determination and program entry.
  • Ensure documentation for every client is recorded in HMIS, our electronic record, within 72 hours and hard copies placed in the file.
  • Complete the needs assessment upon participant’s program entry to connect clients with resources/benefits.
  • Develop case plans with each program participant, reviewing and amending case plan as needed for participant success.
  • Maintain accurate and timely case notes and paperwork in every program case file.
  • Maintaining weekly contact with all program participants. (in-person, email, or phone calls)
  • Review the program budget to ensure proper allocation of funds.

Requirements:

  • Bachelor’s degree in a related field, or a combination of education and experience that demonstrates the ability to perform effective case management or, at least 2 years case management experience.
  • Excellent people skills.
  • Excellent time management skills and ability to multi-task and prioritize work.
  • Attention to detail and problem-solving skills.
  • Excellent written and verbal communication skills.
  • Strong organizational and planning skills.
  • Proficient in MS Office.
  • Knowledge of clerical practices and procedures.
  • Demonstrates the characteristics of a team player and problem solver.

PHYSICAL DEMANDS
Sitting, travel to and from relevant home, office and agency sites.

WORK ENVIRONMENT
The work environment is primarily an office setting but may also include street locations, partner offices, shelters, hospitals, and other agencies located throughout several different counties.

Job Type: Full-time

Pay: $17.12 - $18.47 per hour
