SUMMARY:
Provides general medical care and treatment to clients in UIHS clinics under the direction of collaborative Physician, by performing the following duties. FNPs and Pas fall under the category of Advanced Practice Clinicians (APCs) and will be referred to collectively as such.
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.
- Provides clinical care withing their scope of practice granted via the UIHS credentialing and privileging process.
- Performs physical examinations and preventative health measures withing prescribed guidelines and instructions of Physician.
- Orders, interprets, and evaluates diagnostic tests to identify and assess client’s clinical problems and health care needs.
- Records physical findings, and formulates plan and prognosis, based on client’s condition.
- Discusses case with Physician and other health professionals as needed to prepare comprehensive client care plan.
- Submits health care plan and goals of individual clients for periodic review and evaluation by Physician.
- Prescribes or recommends medications or other forms of treatment such as physical therapy, inhalation therapy, or related therapeutic procedures as prescribed by license and applicable laws and regulations.
- Refers clients to Physician for consultation or to specialized health resources for treatment.
- Attends department and other meetings as directed.
- Adheres to accreditation and compliance standards/guidelines
- Maintains a professional, organized, and clean working environment by following organizational policies, guidelines, and safety standards.
- Assists in development of departmental policy, procedures, and quality improvement activities within clinic as directed.
- Provides after hours back-up phone coverage for client advice by providing at home call coverage for UIHS at least five (5) weeks per calendar year.
- Participates in the peer review process by returning reviews in a timely manner and participating in peer review meetings as needed.
- Records client response and treatment provided in the electronic health record and other appropriate records of care.
- Utilizes good communication skills to maintain positive relationship with clients, providers, co-workers, other departments in the agency, and with other facilities in the community.
- Performs all duties in accordance with the UIHS Mission, Vision, and Guiding Principles.
SUPERVISORY RESPONSIBILITIES:
This job has no supervisory responsibilities although the APC will direct the work of others as it relates to client care.

EDUCATION/EXPERIENCE: Educational degrees must be from a US Department of Education accredited school
Advanced Practice Clinician I:
Be either a Family Nurse Practitioner (FNP) with:
- A valid certificate of completion from an accredited Nurse Practioner Program.
- A current license to practice as a Registered Nurse in the State of California.
- Board Certification as a Nurse Practitioner by a national accrediting agency,
OR A Physician Assistant (PA) with:
- A valid certificate of completion from an accredited Physician Assistant Program.
- A current license to practice as a Physician Assistant in the State of California.
- Board Certification from National Commission on Certification of Physician Assistants (NCCPA)
Advanced Practice Clinician II:
- Meets above minimum qualifications for Advance Practice Clinician I, plus ten (10) years of applicable clinical experience.
COMPUTER OPERATIONS
- Proficient personal computer skills including electronic mail, record keeping, routine database activity, word processing, spreadsheet, presentations.
- Must be able to work in an Electronic Health Records (EHR) system.
- Experience using NextGen preferred.
