JOB TITLE: Homelessness Prevention Case Manager
STATUS: Full-Time
WAGE: $30.00 - $31.20
HOURS: M-F 9:00am – 3:00pm
Job Description
The Homelessness Prevention Case Manager will provide short-term, goal-based case management and advocacy services to individuals and families facing imminent homelessness. This role focuses on creating sustainable housing stability through client empowerment, financial support, and community resource referrals. Under the supervision of the Homelessness Prevention Program Manager, this position fosters collaborative relationships with community partners, schools, and service providers.
Key Responsibilities
Client Services:
- Enroll eligible individuals and families into case management programs.
- Collaborate with clients to identify housing stability needs and establish individualized goals.
- Provide financial assistance and connect clients with appropriate community resources.
Administrative Responsibilities:
- Maintain detailed, accurate records in HMIS and other databases.
- Develop and update forms and documentation in compliance with privacy regulations.
- Attend agency and funder-sponsored meetings and complete required reporting.
Community Engagement:
- Conduct outreach to schools, businesses, and community organizations to promote homelessness prevention initiatives.
- Foster partnerships with landlords, service providers, and other stakeholders to expand resources for clients.
Professional Development:
- Participate in ongoing training, including evidence-based practices like trauma-informed care and motivational interviewing.
- Support training initiatives within the organization as needed.
Other Duties as Assigned
- By Homelessness Prevention Program Manager and Community Services Division Director
- Collaborate with team members to address organizational priorities.
Qualifications
- Proven ability to work both independently and as part of a team.
- Excellent interpersonal and communication skills, with sensitivity to diverse cultural backgrounds.
- Familiarity with local resources and systems addressing homelessness.
- Proficiency in HMIS, Microsoft Office, and case management software.
- Minimum of High School Diploma or GED
Required:
- Proven ability to work independently and in teams.
- Strong organizational skills with attention to detail.
- Excellent verbal and written communication skills.
- Proficiency in Microsoft Office Suite (Word, Excel, PowerPoint, Outlook) and familiarity with project management tools.
- Ability to multitask and prioritize effectively.
- High School Diploma or GED
Preferred:
- Experience in nonprofit organizations.
- Knowledge of or connection to Indigenous communities and cultures.
- Bachelor’s in human services or similar
