Overview:
The Clinical Care Coordinator serves as the primary contact for inquiries, efficiently responding and directing by established procedures and supervisor direction. This role provides administrative support to the HFA Administrator and supervisory staff, emphasizing quality customer service for residents, families, staff, volunteers, visitors, and professionals.
What you’ll do:
Administrative Responsibilities:
- Conduct ongoing chart audits to ensure accuracy and compliance.
- Complete billing processes related to HFA clinical needs.
- Manage access cards for HFA residents and maintain records.
- Generate and analyze call-light reports.
- Provide administrative support in supply ordering, billing, scheduling, time reporting, transcription, and appointments.
Customer Service Responsibilities:
- Communicate necessary resident information to appropriate stakeholders to support effective care and services.
- Facilitate the execution of new physician orders, including documentation when necessary.
- Assist residents and family members with billing inquiries, acting as a liaison between the Business Office and the community.
Clinical Services Responsibilities:
- Support Direct Care Supervisors with pharmacy protocols, including coordinating pharmacy deliveries.
- Arrange diagnostic testing with outside providers and coordinate with residents as needed.
- Manage 360 Care services and schedule resident appointments.
- Assist Direct Care Supervisors during emergency response situations and related tasks.
- Distribute, respond to, and track resident Personal Health Buttons (PHB).
- Maintain and update medical records, ADT reports, and communication logs.
- Participate in reviewing incidents and unusual occurrences (I/UO) as part of a multidisciplinary team for quality improvement.
- Ensure proper signatures are obtained for medication error processes.
- Track I/UO reports and medication error data to support quality assurance and process improvement initiatives.
- Oversee the re-ordering, administration, and tracking of TB tests, flu vaccines, Hepatitis B vaccines, and COVID-19 vaccines.
What it takes:
- Comprehensive understanding of procedures related to medication administration, accurate documentation, and performing and scheduling diagnostic tests.
- A minimum of 2 years of healthcare experience, preferably in geriatrics or aging services.
- Possession of a Medical Assistant Certificate from an accredited school is strongly preferred.
- Ability to safely lift, push, pull, and carry up to 50 pounds. Tasks involving weights exceeding 50 pounds must utilize assistance or appropriate equipment to ensure safety.
