Status: Part Time, 10 hours per week
Schedule: Monday - Sunday | 8a - 4:30pm
Location: Grand Rapids and Surrounding Service Areas
Overview
The Social Worker MSW supports planning, organizing, and implementing social service programs to meet clients' emotional and social needs under the guidance of the Executive Director.
Responsibilities:
- Plan, develop, implement, evaluate, and manage social services for the agency.
- Participate in community planning to support the needs of residents and families.
- Assist with discharge planning, social care plans, and resident assessments (e.g., psychosocial assessments, MMSE, GDS, MDS, RAPs, PAS/ARR).
Communication:
- Demonstrate effective written and verbal communication skills.
- Show cultural sensitivity when interacting with diverse groups.
Direct Care:
- Work independently with flexibility, integrity, and professionalism while assisting older adults and diverse populations.
- Perform physical tasks, including lifting, carrying, pushing, and pulling as required for resident care (e.g., transfers, wheelchair assistance).
- Walk, stand, and perform repetitive tasks for extended periods.
Customer Service:
- Collaborate as part of a team to achieve goals.
- Offer assistance to residents, staff, and guests with a helpful attitude.
Ethics, Confidentiality, & Integrity:
- Maintain confidentiality of individuals served, staff, and company information.
- Operate within the scope of the role and comply with laws and accrediting standards.

What it takes
- Must possess a master’s degree in social work.
- Must have a minimum of 2 years of supervised social work experience in a health care setting working directly with individuals
