Care Navigator:
Location: Dayville, CT
United Services is looking to hire a full-time Care Navigator in our Comprehensive Services Division. Comprehensive Services’ goal is to help transform community behavioral health systems and provide comprehensive, coordinated behavioral health care and medical care. The focus is on integrating care throughout the behavioral and medical health system to benefit individuals meet their recovery goals. Comprehensive Services focuses on delivering person-centered care.

Care Navigators will provide care management, care coordination, individual and family support, hospital transition planning and health promotion services to those that are enrolled in Comprehensive Services. The Care Navigator coordinates the person centered care plan, provides ongoing care coordination and treatment planning, tracks and performs required assessments and screening, monitors outcome measures.
Current positions available in Mansfield CT & Dayville
Care Navigator Schedule: Monday - Friday 8:30 am - 5:00 pm.
Care Navigator Salary: $18.54 - $20.46
Bilingual Spanish Fluency: $19.48 - $22.04
Care Navigator Requirements:
Education: Bachelor’s Degree in relevant human services field or Associate’s Degree in relevant human services field plus two years of relevant experience.
