Hospital Liaison Description:
United Services is seeking a hospital liaison to provide a higher level of care coordination to the clients we serve. The qualified candidate will provide outreach, engagement, and coordination as assigned by the program manager to at-risk or difficult-to-engage clients and represent USI at external meetings to coordinate client care, including dispositions of client care. As a Liaison at United Services, Inc., you will play a vital role in connecting individuals with the resources they need for mental health and wellness. This position is essential for fostering relationships within the community and ensuring that our services are accessible to those who require support.
Responsibilities
- Establish and maintain relationships with community organizations, healthcare providers, and stakeholders.
- Facilitate communication between clients and service providers to ensure seamless access to care.
- Assist in coordinating care plans for clients, ensuring their needs are met effectively.
- Promote United Services’ programs within the community to raise awareness about available resources.
- Gather feedback from clients and stakeholders to improve service delivery.
- Participate in outreach activities to engage potential clients and partners.
Requirements
- Strong organizational skills with the ability to manage multiple tasks effectively.
- Excellent verbal and written communication skills for effective interaction with diverse populations.
- Prior experience in care coordination, customer service or account management is highly preferred.
- Familiarity with behavioral health services or social work is a plus but not required.
- Proficient in Microsoft Office Suite, particularly Word.

Job Type: Full-time
Pay: $18.54 - $20.98 per hour
Expected hours: 40 per week
