NURSE CARE MANAGER DESCRIPTION
United Services is seeking to hire a full-time Nurse Care Manager (LPN/RN) for our Behavioral Health Home. If you are passionate about working with individuals with behavioral health diagnoses and chronic health conditions with a team approach and are looking for a supportive work environment that encourages a good work-life balance, we are looking for you! The Nurse Care Manager will provide care management, care coordination, individual and family support, hospital transition planning, and health promotion services to Behavioral Health Home enrollees and Comprehensive Services clients.
United Services, Inc. requires all employees to be vaccinated against influenza annually during flu season.
Nurse Care Manager Schedule: Monday through Friday from 8:30 am to 5 pm, with no evening, weekend, or holiday required.
Nurse Care Manager Salary range: Competitive Salary
Nurse Care Manager Requirements:
Current Registered Nurse (RN) / Licensed Practical Nurse (LPN) license. Able to maintain and renew licensure as RN/LPN in Connecticut.
