Nurse Care Manager: United Services is looking to hire full-time Nurse Care Managers (LPN/RN) for our Behavioral Health Home and our Comprehensive Services Division. If you are passionate about working with individuals with behavioral health diagnoses and chronic health conditions with a team approach and are looking for a supportive work environment that encourages a good work-life balance, we are looking for you! The Nurse Care Manager works Monday-Friday 8:30am-5pm with no evening, weekend or holidays required. This is a great opportunity to work with individuals to meet their health and wellness goals, provide person-centered care, provide care coordination to have a positive impact on one’s life.
Nurse Care Managers will provide care management, care coordination, individual and family support, hospital transition planning and health promotion services to Behavioral Health Home enrollees and Comprehensive Services clients.
United Services, Inc. requires all employees to be vaccinated against influenza annually during flu season and against COVID-19 prior to start date.
Nurse Care Manager Schedule: Monday - Friday, 8:30 a.m. - 5:00
