What's your day like?

As the Full-Time Manager of Community Connectivity, you will play a pivotal role in managing place-based Social Navigators while also supporting the development and co-leading the implementation of a Social Navigation Community of Practice. This innovative space will bring together human services change agents from various sectors and disciplines, fostering collaboration and shared learning. By regularly convening case managers across the region, this community will create opportunities to discuss emerging trends, share valuable outcomes and resources, and exchange best practices for supporting clients in navigating the complex human services landscape. This dynamic environment will empower professionals to reach common goals, driving health equity and enhancing the effectiveness of social services throughout our community. Your leadership will be crucial in ensuring that this community becomes a beacon of collaboration and innovation within nonprofit management.

Requirements for this Manager, Community Connectivity job

To excel as the Full-Time Manager of Community Connectivity, you will need a robust skill set that combines experience and knowledge in human services and nonprofit management. A minimum of three years in community outreach and engagement is essential, with demonstrated success in working alongside communities to achieve impact goals. Your experience in project and program design, implementation, large group facilitation, and management in a social sector setting will be critically important. Familiarity with health equity and the social determinants of health is necessary, as well as a proven ability to embed equity practices into program frameworks and outcomes. To be most successful, this candidate must have the ability to consult, connect, facilitate and collaborate with community partners in service of creating and evaluating shared impact goals using a Community of Practice framework.

Additionally, proficiency in using various resource databases and customer service tools, including software such as iCarol, Navigate, and HMIS, will be vital for effective contact management. A bachelor's degree in human or social sciences, public administration, or a related field, or equivalent experience, will further bolster your qualifications for this impactful role.

Knowledge and skills required for the position are:

  • At least three years of experience in community outreach and management with documented evidence of working alongside community towards an impact goal
  • At least three years of experience in project and/or program design implementation and management. Social sector setting is strongly preferred
  • Prior knowledge of health equity and social determinant of health impact Frames.
  • Proven track record of embedding equity practices into program design implementation and outcomes
  • Previous experience using resource databases customer service tools or contact management software such as iCarol, Navigate, HMIS, AIRS, RELIAS, Service Point, EPIC, inContact, MI Bridges or other customized software
  • Bachelor's degree in human or social sciences nonprofit management public administration education business; or equivalent experience in a related field
