General Description: Under general supervision, the incumbent will support all elements of the MISSION UNITED programs and initiatives, including serving as the primary point of entry for clients accessing the appropriate staff or resource. The purpose of the position is to support the health and wellbeing of Veterans, Service Members, and their families through accurate, timely, and successful linkage to resources throughout North Texas.
Supervisory Responsibilities: None
Primary Job Responsibilities & Duties:
- Implement and maintain designated processes in assisting clients including, but not limited to the following:
- Receive and review incoming self-referral
- Assist in the assignment of referrals
- Conduct intake to collect necessary data, documents, and qualifying information
- Receive and review incoming referrals from network providers, collect necessary data from providers and prepare to send referrals out to network providers
- Identify appropriate network providers for referrals and send referrals in key areas (e.g., housing, employment, financial assistance, benefits, social connections, etc.)
- Follow-up with clients for additional information as needed
- Document, track, and follow up on referral progress
- Conducts assessments and completes intakes or makes community referrals
- Identification of clients, individual assessment/screening, individual service plan development, arranging necessary care and services, follow-up, and ongoing monitoring of consumer status
- Accurately enter all intake information into client management information system
- Accurately interpret and assess client needs based on client verbalizations and follow-up questions
- Meet designated benchmarks and/or quality measures
- Create and present reports to providers and staff
- Conduct all work and documents activity in accordance with partner agreements and UWTC policies and procedures
- Additional duties as assigned
Other Job Duties:
- Identify gaps in service provision within the community
- Serve as a customer service role model to ensure a positive relationship in interactions with all clients, the community, and peers
- Perform duties as needed and/or requested to support and maintain standards and continuous quality improvement of MISSION UNITED and United Way of Tarrant County
- Must be able to work evenings/weekends and maintain reliable transportation
- Attend local and regional meetings
- Must be able to be at work on a consistent basis with regular attendance, punctuality and be willing and able to handle all the duties and responsibilities of this position daily
- Respond appropriately and effectively to the diversity of co-workers, consumers, and stakeholders
- Take responsibility for other assignments as they arise in the development of MISSION UNITED activities
- Coordinate planning and requests from other Divisions that relate to MISSION UNITED
- Due to the leadership role United Way is called upon to play in the community, particularly during times of crisis, it is the expectation that all United Way staff will be fully engaged in the organization's crisis plan and response efforts
- Staff liaison for MISSION UNITED’s engagement, including recruiting volunteers, leading volunteer training sessions, initiating annual strategic planning, developing engagement opportunities, coordinating with UWTC Marketing team, and overseeing execution of activities
Qualifications
Essential Knowledge, Skills and Abilities:
To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required.
- Must be eligible to work in the United States (No Sponsorship)
- Knowledge of local service delivery system and community resources
- Personal knowledge of veterans’ experiences and/or issues and abilities strongly preferred
- Proficiency in MS Office and experience/ability to learn software packages and client service tracking systems
- General knowledge of office systems and duties including use of scanners and copy machines
- Strong interpersonal, written, and verbal communications skills, including ease in public speaking
- Ease in creating rapport with new relationships
- Analytical skills, professional acumen, business ethics, continuous improvement processes, problem solving skills, and respect for confidentiality of information
- Fluency in Spanish Preferred
Education & Experience:
- High School Diploma/GED required and an associate or bachelor’s degree in a social work, psychology, or a related human services field preferred
- Knowledge of general interviewing practices and/or techniques; including crisis prevention and intervention preferred
- Experience in social work or related human services field, or significant experience in customer service strongly preferred
- Proven ability to work effectively independently, in teams, and with people of diverse backgrounds.
- Veteran or experience working with veterans strongly preferred
- Proficient with Microsoft Office Suite
Work Environment:
The work environment characteristics described here are representative of those an employee encounters while performing the essential functions of this job. Although remote work is permitted for this position, the individual must reside primarily in North Texas with the ability to work from the office or other necessary location on a daily basis if needed.
Additional requirements related to the job: Must have a valid Texas driver’s license, proper vehicle insurance and use of vehicle to travel throughout the area
