Description
Full-Time, 10 days per pay, 3pm-11:30pm, weekend and holiday rotation.
Job Summary: Oversee nursing activities and assignments; organize and assign responsibilities based on resident population, staff qualifications, and physical structure. Oversee and monitor Personal Care team in resident care based on job requirements and resident expectations.
Reports to: Director of Personal Care
Primary Essential Responsibilities:
- Resident Rounds: Regularly check on residents, administer medications, provide treatments, consult with physicians, and respond to emergencies.
- Regulatory Compliance: Ensure care and services meet all regulatory requirements.
- Ethical Leadership: Demonstrate ethical leadership, exemplifying the UZRC Core Values.
- Team Success: Facilitate team members’ success through feedback, goal setting, coaching, and coordinating necessary training.
- Policy Enforcement: Observe, interpret, apply, and enforce policies and procedures.
- Training: Assist in staff training, problem-solving, and maintain records of residents’ conditions, medications, and treatments.
- Communication: Regularly communicate with the Director of Personal Care and all interdisciplinary team members.
- Promote Independence: Encourage resident participation in care and decision-making.
- Staffing: Ensure adequate staffing for all shifts.
- Service Continuity: Ensure continuity of service delivery across shifts.
- Problem-Solving: Demonstrate proactive problem-solving skills in crisis situations.
- Performance Evaluations: Assist Director with timely staff evaluations to ensure accountability.
- Resident Care: Provide care to residents, administer medications, provide treatments, assess residents, and document appropriately.
- Leadership: Direct Personal Care team to ensure resident needs are met, and participate in performance improvement processes.
- Resident System Assessment: Monitor residents for changes, document and report to Director
- Documentation: Complete all required documentation for residents, including test results, lab work, medication administration, and physician orders.
- Care Planning: Work with Director in the development of individual resident care plans, assess needs, recognize changes, and update forms timely.
- Improvement Opportunities: Identify opportunities to enhance care delivery and policy alteration.
- Training: Attend all required in-service training, department meetings, all team meetings and maintain nursing skills, participate in additional in-services. Timely completes all required training.
- Customer Service & Compliance: Provide excellent customer service, maintain integrity, adhere to all United Zion policies.
- Participate in on call rotation.
- Time Management skills, quickly adapt to changes, be mentally alert.
- Flexible, caring, and ability for harmonious working relationships.
- Basic knowledge of, an ability to use EMR.
Qualifications
Position Qualifications:
- Education: High school diploma or equivalent.
- Language Proficiency: Must be proficient in reading, writing, and speaking English.
- Licensed Practical Nurse by Board of Examiners in PA, in good standing.
- Completed pharmacology course.
- Current CPR certification.
